齐拉西酮与氯丙嗪对首发精神分裂症病人认知功能影响比较

摘　　要：目的比较齐拉西酮和氯丙嗪对首发精神分裂症病人认知功能的影响。方法将符合入组标准的首发精神分裂症病人72例随机分为齐拉西酮组与氯丙嗪组,分别进行8周的系统治疗,使用阳性与阴性症状量表（PANSS）、数字划销测验（CT）、韦氏成人记忆量表（WMS-RC）和威斯康星卡片分类测验（WCST）进行检查,评估其疗效和对认知功能的影响。结果脱落3例,两组病人治疗8周后PANSS总分较治疗前明显下降（t=5.08、5.11,P〈0.01）,但两组之间治疗后PANSS总分比较差异无显著性（P〉0.05）。治疗后齐拉西酮组CT中的划对数目、划漏数目、净分值,WMS-RC中的记忆商数,WCST中的总测验次数、分类完成数、持续性错误数与氯丙嗪组比较差异均有显著性（t=-5.82~2.65,P〈0.05）。结论齐拉西酮对首发精神分裂症病人认知功能的影响明显好于氯丙嗪。Objective To compare the effects of ziprasidone and chlorpromazine on cognitive function in the first-onset schizophrenic patients. Methods Seventy-two patients who met the standards of initial schizophrenia were randomized to two groups receiving ziprasidone and chlorpromazine therapy, respectively, for eight weeks. The therapeutic response and cognitive function were assessed by positive and negative symptoms scale （PANSS）, the cancellation test （CT）, Weschler memory scale （WMS-RC） and Wisconsin Card Sorting Test （WCST）. Results Of the 72 patients, three discontinued treatment. The total score of PANSS in the rest 69 patients, in two groups, who completed the course of therapy was significantly lower than that before treatment （t= 5.08,5.11;P〈0.01）, but the difference of that between the two groups was not significant （P〉 0.05）. The accurate cancellation, missed cancellation, total score of CT, memory quotient of WMS-RC and total trials, categories, durative errors of WCST were significantly different between the two groups （t=-5.82--2.65,P〈0. 05）. Conclusion The effect of ziprasidone on cognitive function in first-onset schizophrenia is better than that of chlorpromazine.
