雄激素受体表达影响胃癌患者术后微转移

摘　　要：目的:探讨雄激素表达不同的男性胃癌患者外周血微转移情况。方法:选择男性胃癌患者60例,根据雄激素表达情况分为阳性组与阴性组,分析两组胃癌患者年龄、病变浸润深度、分化程度和大体分型等临床特征,以及手术时间和手术术式等术中情况,并检测所有患者外周血CEA-mRNA水平。结果:阳性组与阴性组男性胃癌患者年龄、侵犯深度、大体病理类型等临床特征,以及手术时间和手术方式差别均不显著(P>0.05)。阳性组男性胃癌患者与阴性组比较,低分化程度比例高(P<0.05),外周血CEA-mRNA阳性率高(P<0.05)。有意义的危险因素OR值分别为:手术时间(OR=2.148,95%CI1.785～2.563);侵犯深度(OR=1.268,95%CI1.040～1.533);分化程度(OR=0.804,95%CI0.577～1.103);雄激素表达情况(OR=0.652,95%CI0.471～0.883)。结论:雄激素表达情况与胃癌患者外周血微转移有关。Objective： To study the effects of androgen receptor expression state on the expression of serum CEA-mRNA in male patients with gastric carcinoma. Methods： 60 male patients with gastric carcinoma were divided into positive group and negative group by the expression state of androgen receptor. The clinical feature of two groups, such as age, invasion depth, differentiation state and Brrmann category, and the operation information, which include operation time and operation type, were all investigated. Then the serum level of CEA-mRNA were detected. Results： The clinical feature（such as age, invasion depth and Brrmann category） and the operation state（operation time and operation type） between the positive group and the negative group were similar（P〉0.05）. The rate of the poorly differentiated and the positive rate of peripheral blood CEA-mRNA in patients in the positive group were all higher than that in the negative group （P〈0.05）. When all the patients were analyzed by means of logistic regression analysis, the adjusted OR of given risk factors for micrometastasis in male patients with gastric carcinoma were 1.268 （invasion depth, 95%CI 1.040-1.533）; 0.804 （differentiated level, 95%CI 0.577-1.103）; 0.652 （positive expression of androgen receptor, 95%CI 0.471-0.883）; 2.148 （operation time, 95%CI 1.785-2.563）. Conclusion： The positive expression of androgen reeepto is a risk factor for peripheral blood micrometastasis in male patients with gastric carcinoma.
