摘 要:目的探讨失三叉神经支配对兔下颌骨牵张成骨新骨形成的影响。方法对20只新西兰白兔实施双侧下颌骨牵张成骨,左侧在手术显微镜下暴露并切取约0.6cm下牙槽神经,保护下牙槽血管束,右侧仅暴露下牙槽神经而不切断。术后在固定期14d、28d取材,对新生骨痂进行HE染色及骨组织计量学观察。应用SPSS12.0软件包进行配对t检验。结果实验侧牵张间隙骨小梁分布方向较杂乱,骨小梁内部有分散的软骨岛分布;对照侧牵张间隙骨小梁主要沿牵张方向分布,血管丰富,成骨细胞活跃。计量学分析显示在固定期14d、28d时,离断下牙槽神经侧新生骨量百分比分别较对照侧低23.1%、17.3%(P<0.05),新生骨小梁宽度分别较对照侧低14.0%、13.4%(P<0.05)。结论离断下牙槽神经可影响下颌骨牵张成骨新骨形成,提示感觉神经系统在牵张成骨过程中起到一定调控作用。Objective To investigate the effects of inferior alveolar nerve resection on new bone formation in rabbit mandibular distraction osteogenesis. Methods A total of 20 New Zealand white rabbits underwent bilateral distraction osteogenesis with a rate of 1 mm/day. The inferior alveolar nerve of one side was resected under the surgical microscope, while the inferior alveolar vessel bundle was protected carefully. The contralateral side received sham operations. The rabbits were sacrificed at consolidation time of 14 and 28 days, and then the regenerated callus underwent HE staining and histomorphometric analysis. A paired t test was performed using SPSS 12.0 software package. Results Histological investigation showed that the bone trabeculae were disarrayed containing dispersed cartilage cells in the denervated side, whereas the bone trabeculae disarryed orderly with rich blood vessels and no cartilage cell in the control side. Histomorphometric analysis demonstrated that both new bone volume (NBV) and the thickness of new trabeculae (TNT) were significantly lower in the denervated side than in the control side (P 〈 0.05). Conclusion The inferior alveolar nerve resection can result in less and disarrayed new bone trabeculae with increased cartilage cells, which suggests that the peripheral nerve plays a role during the new bone formation in the distraction osteogenesis.
