摘　　要：目的探讨鼻肠减压管在中晚期恶性肿瘤性小肠梗阻治疗中的综合应用价值。方法18例中晚期恶性肿瘤患者发生小肠梗阻时于DSA透视下插入鼻肠减压管至十二指肠屈氏韧带附近,观察患者鼻肠减压管插入后近期临床表现及导管引流情况,并于导管停止移动后经导管行肠内造影以明确诊断,其中15例患者在腹胀缓解后予口服supportan治疗。结果18例患者鼻肠减压管均顺利插入预定位置,在术后2d至1周内肠梗阻症状基本缓解;经导管肠内造影,18例患者均有阳性征象;15例患者口服supportan2周后营养情况改善。结论在中晚期恶性肿瘤性小肠梗阻综合治疗中鼻肠减压管插入相对简单,综合治疗效果明显,值得采用。Objective To investigate the clinical application of naso-intestinal decompression intubation in the comprehensive treatment of small intestine obstruction caused by advanced malignant tumors. Methods Under DSA guidance, naso-intestinal decompression intubation was performed in 18 patients with small intestine obstruction caused by advanced malignant tumors. The naso-intestinal decompression tube was inserted into the small intestine and its tip was placed in the vicinity of ligament of Treitz. The patients＇ clinical manifestations and the drainage condition were observed and recorded. Contrast media was injected through the tube to clarify the diagnosis. Oral administration of supportan was applied in 15 patients after abdominal distension relieved. Results The naso-intestinal decompression tube was successfully inserted to scheduled position in all 18 patients and the symptoms of intestinal obstruction were basically alleviated in 2- 7 days after the procedure. Intestinal contrast examination showed positive imaging signs. The malnutrition condition was obviously improved in 15 patients after orally taking supportan for two weeks. Conclusion DSA-guided naso-intestinal decompression intubation is a technically simple and clinically effective method for the treatment of small bowel obstruction caused by intraperitoneal metastases, therefore, it is worthy of clinical use.
