摘  要:目的探讨内皮生长因子(VEGF)和微血管密度(MVD)表达在早期周围型肺影像学诊断中的应用价值。方法对68例经手术病理证实的周围型肺癌(腺癌32例,鳞癌30例,小细胞癌6例)采用LDP免疫组化法,检测肿瘤标本中VEGF和MVD的表达,并分析其与影像学征象之间的关系。结果VEGF和MVD值呈正相关关系(r=0.691,p<0.01),VEGF和MVD表达与周围型肺癌的组织学类型无关(p>0.05),与肿瘤的大小、分叶征、棘状突起、胸膜凹陷征、血管集束征、纵隔淋巴结转移及胸腔积液等影像学征象均有关(p<0.05),而与毛刺征无关(p>0.05)。结论周围型肺癌的影像学征象与VEGF和MVD表达密切相关,当肿瘤直径大于3cm,出现分叶征、棘状突起、胸膜凹陷征、血管集束征、纵隔淋巴结转移及胸腔积液等征象,提示肿瘤可能有较高的恶性程度。Objective To investigate endothelial growth factor (VEGF) and microvessel density (MVD) in the early expression of peripheral lung imaging diagnosis. Methods 68 cases confirmed by surgery and pathology of peripheral lung cancer (32 cases of adenocarcinoma, 30 cases ofsquamous cell carcinoma and small cell carcinoma 6) LDP used immunohistochemistry to detect tumor samples MVD and VEGF expression and analysis of its operation Pre-performance imaging. Results VEGF and MVD value of a positive correlation (r = 0.692, p 〈0.01), VEGF and MVD expression of peripheral lung cancer histological type (p〉 0.05), and the size of the tumor, Iobulation, spine-like processes, pleural Depression symptoms, vascular convergence sign, mediastinal lymph node metastasis and pleural effusion, and other imaging signs are related (p 〈0.05),but the levy had nothing to do with the burr (p〉 0.05). Conclusion Peripheral Lung Cancer Imaging and signs of MVD and VEGF expression are closely related to, when the tumor diameter greater than 3cm, appeared lobulation, spine-like processes, pleural indentation, vascular convergence sign, mediastinal lymph node metastasis and pleural effusion, such as signs, suggesting that tumor may have a higher degree of malignancy.
