老年急性脑出血患者血清C反应蛋白水平及其动态变化

摘　　要：目的研究老年急性脑出血患者血清C反应蛋白含量的动态变化及其与出血体积、预后的关系。方法将44例急性脑出血患者,均于出现症状24h内入组,40例健康对照者来自本院健康体检中心。在入院时及第1、2、3、4、6、10天各留取血液标本1次,采用免疫散射比浊法测定C反应蛋白。出血体积为CT经Simes Somatom所提供的软件测得。神经功能恢复评定标准为修正的Rankin等级评分。结果C反应蛋白水平第2天达到高峰,从发病开始的C反应蛋白水平均与最终的脑出血体积存在正相关关系,第2天相关关系最为密切(r=0.781,P<0,001)。且与Rankin等级评分(r=0.653,P<0,001)相关。结论血清中C反应蛋白水平含量的测定可能反映急性脑出血患者的出血体积,还可以预测患者的预后。Objective To investigate the correlation of serum C-reactive protein concentrations with hemorrhage volume in acute cerebral hemorrhage. Methods Forty-Four patients in our Hospital with acute cerebral hemorrhage presenting less than 24 hours after symptom onset were incorporated intothe study.Serum CRP protein levels were determined on hospital admission and 1,2,3,4,6 an d 10 days after symptom onset.Functional outcome in 3 months after stroke（modified Rankin scale score）and final hemorrha volume on day 7 were determined by means of standardized volumetry of brain images. Resealts The CRP concentration reached a maximun on day 2.The level of CRP on day 2 was well correlated with the hemorrhage volume （r=0.781,P〈0.001）,and the level of CRP on day 2 Was also well correlated with the hemorrhage volume （r=0.937.P〈0.001）and with the clinical outcome assessed with modified Rankin scale score（r=0.653,P〈0.001）. Conclusion CRP in blood during acute stroke are useful markers of hemorrhage volume,and they may serve as easily accessible,inexpensive and valid prognostic markers in the future.
