摘 要:目的观察超声心动图对先天性右位心合并复杂畸形的诊断价值。 方法以手术结果为“金标准”,心血管造影结果为对照,应用超声心动图按VanPraagh节段分析法评价62例先天性右位心合并复杂心血管畸形患者的心脏解剖结构。 结果62例右位心患者中镜像心28例,右旋心34例。超声心动图和心血管造影对各节段诊断特异性有高度一致性,两方法相比较无显著性差异(P〉0.05)。超声心动图对心房方位、心室形态及房室连接、心室动脉连接及主动脉位置诊断准确性均大于91.3%,但对心房异构及某些动静脉畸形诊断较困难。 结论超声心动图对右位心合并复杂心血管畸形患者大多能作出明确诊断,可作为右位心诊断的首选或筛选方法。Objective To evaluate the echocardiographic diagnostic value of congenital dextrocardia in morphologic characterization and cardiovascular anomalies. Methods The structure of dextroeardia with the cardiovascular anomalies, based on operation, were determined by echocardiography according to Van Praagh's methods compared with angiography. Results Of total 62 patients,28 cases had mirror-image dextrocardia,34 cases had a dextroversion. There have high consistency and no difference in diagnosing dextrocardia with complex anomalies between two methords-echocardiography and angiography( P2〉0. 05), the specificity of transthoracic echocardiography for diagnosing the atrial situs,ventrieular structure, or the connection of the ventricle and arteries, arterial situs was more than 91.4%, but sometimes it would be difficult to detect some extracardiac malformations accurately and aerial situs in patients with atrial isomerism. Conclusions Dextrocardia, presented with abnormal segmental connections and complex anomalies, could be diagnosed by echocardiography which could be the first screening choice for diagnosing dextroeardia.
