肠内容物特性对胶囊内镜图像质量的影响

摘　　要：目的了解小肠肠内容物对胶囊内镜图像质量的影响。方法对我科进行胶囊内镜检查的56例受检者随机分为肠道准备组和禁食组。肠道准备组在检查前使用硫酸镁法(70%MgSO470ml)进行肠道清洁并服用二甲基硅油2ml,禁食组只在检查前禁食8h。对小肠内气泡量、肠液量及清洁度、观察效果进行定量分析。结果肠道清洁后,小肠内气泡的形成与小肠观察效果均有明显的改善(P<0.05),但对肠液量和清洁度无明显的影响(P>0.05)。禁食状态下,肠道内气泡量和肠液清洁度是胶囊内镜对小肠观察效果的独立影响因子;肠道清洁处理后,肠液清洁度和肠液量则为独立影响因子。结论不同肠道准备状态下,各因素对胶囊内镜的观察效果起到了不同的作用。改善肠液清洁度是提高胶囊内镜图像质量的关键。Objective To learn the effect of intestinal content on the image quality of capsule endoscopy. Methods Fifty-six patients who underwent capsule endoscopy were divided into two study groups：bowel preparation group（n=28）accepted oral bitter salt 4h prior to capsule endoscopy;and fasting group （n=28）accepted an overnight fast, The image quality of capsule endoscopy were graded according to air bubble size, digestive tract juice quantity and its cleaning grade. Results The bowel preparation group presented significantly better quality of images compared to fasting group（P〈0.5） ,and the same as air bubble too. But the grade of cleaning and volume of intestinal content were not affected by the use of bitter salt（P〉0.05）. As fasting,the grade of cleaning of intestinal content and the volume of air bubble were the independent factors that affected the image quality But as bowel preparation, the independent factors were the grade of cleaning and the volume of intestine content. Conclusion The grade of cleaning of intestinal content is the key that affect image quality of capsule endoscopy. Oral purgatives is a sound modality that can improve the quality of intestinal content.
