摘　　要：目的探讨PEIT和PHDI治疗HCC的临床应用价值。方法将40例HCC患者随机平均分为两组,两组在病情、病程、分类、年龄、性别、肿瘤大小和数量等方面无明显差异。两组患者分别接受PEIT及PHDI治疗,所有病例于治疗前后定期行彩超检查,同时检测肝功能、AFP、AFP mRNA、体液介质及CD4。结果PEIT组治疗后肿瘤直径缩小有效率高于PHDI组,差异有统计学意义(P<0.05);两组治疗前后体液介质、CD4、AFP、AFP mRNA差异均有统计学意义(P<0.05),PHDI组AFP降至正常者及AFP mRNA转阴率与PEIT组比较差异有统计学意义(P<0.05);PEIT和PHDI治疗后谷丙转氨酶升高分别为75%及5%,局部复发分别为16.67%及7.14%。结论PEIT和PHDI治疗HCC疗效稳定可靠。Objective To explore the clinical value of ultrasound guided percutaneous ethanol injection and hyperthermal distilled-water injection in treatment for HCC. Methods Forty patients with HCC were randomly devided into two groups. There was no significant difference of age, sex, disease condition, course, size and number of tumor between two groups （P〉0.05）. Patients in PEIT group were performed by percutaneous ethanol injection （PEIT）, and patients in PHDI group by percutaneous hyperthermal distilled-water injection （PHDI）. All patients underwent color Doppler sonography before and after treatment, and liver function, AFP, AFPmRNA, CD4 and body fluid medium （sIL-2R, IL-8, TNFa） were examinated at the same time. Results The rate of tumor regression in PEIT group was significantly higher than that of PHDI group （P〈0.05）. There was significant difference of AFPmRNA, AFP, CD4 and body fluid medium （sIL-2R, IL-8, TNFa） level in both group before and after treatment （P〈0.05）. The change of AFP and AFPmRNA were significant between the two groups （P〈0.05）. After treatment, GPT increased in 75% patients of PEIT group and in 5% patients of PHDI, and the local recurrence rate was 16.67% and 7.14% respectively. Conclusion PEIT and PHDI are effective and reliable treatment for HCC.
