cFLIP与HPV-16 E7在宫颈组织中的表达及其相互关系

摘　　要：目的检测宫颈病变组织中cFLIP与人乳头瘤病毒-16E7(HPV-16E7)的表达,并研究二者在宫颈组织恶性转化过程中的作用及其相互关系。方法应用免疫组化SP染色法检测40例宫颈癌、40例宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)及20例宫颈炎组织中HPV-16E7与cFLIP的表达情况。结果cFLIP在宫颈癌、CIN及宫颈炎组织中的阳性表达率分别为92.50%、70.00%及30.00%,两两比较差异均有统计学意义(均P<0.05);3种宫颈组织中HPV-16E7的阳性表达率分别为82.50%、75.00%及60.00%,两两比较差异均无统计学意义(均P>0.05)。cFLIP、HPV-16E7表达阳性率与宫颈癌病理类型、组织学分级、淋巴结转移及临床分期无明显关系(均P>0.05)。cFLIP表达与HPV-16E7表达呈正相关(r=0.268,P=0.02)。结论cFLIP在宫颈癌组织中特异性高表达,可能通过抗凋亡作用及逃避免疫监视影响机体对HPV感染的免疫反应,并可能协同HPV-16E7影响细胞生长周期,在宫颈癌的恶性转化过程中共同促进肿瘤的发生、发展。Objective To investigate HPV-16 E7 and cFLIP expression in cervical diseases and to further explore the role of HPV and cFLIP in carcinogenesis of cervical cancer. Methods HPV-16 E7 and cFLIP expression was detected by using SP immunohistochemical methods on paraffin-embedded sections from 40 cases of cervical cancer, 40 of cervical intraepithelial neoplasia （CIN）, and 20 of cervicitis. Results The positive rate of cFLIP expression in cervical cancer, CIN and cervicitis was 92.50%, 70.00% and 30.00% respectively （P〈0.05）, and that of HPV-16 E7 was 82.50%, 75.00%, and 60.00% respectively （P〉0.05）. The expression of cFLIP in cervical cancer had no correlation with clinical stage, pathologic classification, histological grade and lymph node involvement （P〉0.05）. cFLIP expression was positively correlated to HPV16 E7 infection （r=0. 268, P=0.02）. Conclusion Over-expression of HPV-16 E7 and cFLIP may be involved in the development and progression of cervical cancer, cFLIP may interact whith HPV-16 E7 through anti-apoptosis, immune escape and cell cycle change during the transformation of CIN into cervical cancer.
