摘　　要：目的研究非甾体类抗炎药(NSAIDs)致上消化道出血患者的临床特点。方法收集412例经胃镜及实验室检查证实上消化道出血患者,其中98例出血前10d内有服用NSAIDs史,分为老年组(122例)和中青年组(290例)进行研究。结果老年组NSAIDs相关性上消化道出血发生率明显高于中青年组(P<0.01);老年组服用药物以阿司匹林为主,中青年组以布洛芬为主;伴随疾病老年组以心脑血管疾病为主,青年组以上感或关节疾病为主;老年组消化道出血以溃疡性病变为主,中青年组以黏膜糜烂为主。Hp感染老年组略低于中青年组差异无统计学意义(P>0.05),但2组内NSAIDs相关性与非相关性比较,差异有统计学意义(P<0.01)。结论老年人患NSAIDs相关性上消化道出血具有其自身特点,不同于中青年患者,对于长期服用NSAIDs的老年患者应加强监测。Objective To study the clinical features of the NSAIDs-induced upper gastrointestinal bleeding. Methods To collect 412 cases confirmed that patients with upper gastrointestinal bleeding by endoscopy and laboratory tests, 98 cases of which within the 10 days before bleeding used NSAIDs, divided into and young and middle-aged study group. Results NSAIDs-related upper gastrointestinal bleeding of older-aged group were significantly higher than young and middle-aged group （ P 〈 0.05 ） ; older-aged group group-based drugs to aspirin, young and middle-aged group used mainly ibuprefen; The older- aged group have with the cardiovascular and cerebrevascular disease-oriented, and uuper respiratory tract infection or arthritis disease to the youth group ; old age group with gastrointestinal bleeding ulcerative lesions mainly , young and middle-aged group mainly mucosal erosion; Hp infection in young and middle-aged group was slightly lower than the elderly group （P 〉 0. 05 ）, two groups of Hp infection rates are lower than the group without using NSAIDs （ P 〈 0.01 ）. Conclusion The elderly people suffering from NSAIDs-related upper gastrointestinal bleeding has its own characteristics, different from the young and middle-aged patients, for long-term NSAIDs use in elderly patients should be strengthened to monitor.
