摘 要:目的:探讨激肽释放酶8(KLK8)在卵巢癌中的表达及其意义。方法:免疫组化法检测卵巢上皮性肿瘤组织中KLK8蛋白的表达水平,其中良性卵巢肿瘤20例、交界性卵巢肿瘤11例、卵巢癌62例,并测量其平均灰度值(A值);酶联免疫吸附(ELISA)双抗体夹心法检测血清KLK8浓度;分析卵巢癌组织中KLK8表达的A值与血清浓度值之间的相关性,比较血清KLK8、CA125检测用于卵巢癌诊断的敏感度及特异度。结果:(1)良性卵巢肿瘤、交界性卵巢肿瘤及卵巢癌中KLK8阳性表达率分别为25%(5/20)、27.3%(3/11)及66.1%(41/62),卵巢癌组阳性表达率明显高于前两组(P<0.05)。在不同临床病理特征间,差异亦有统计学意义(P<0.05);(2)血清KLK8浓度分别为4.26±0.29、5.26±0.46、6.59±0.15μg/L,差异有统计学意义(P<0.01);(3)卵巢癌组织KLK8表达的A值为156.4±14.7,与血清浓度值呈显著正相关,Spearman等级相关系数为0.608(P<0.001);(4)KLK8用于卵巢癌诊断的敏感度61.3%,特异度77.4%,与CA125差异无统计学意义(P>0.05)。结论:卵巢癌组织及血清中KLK8蛋白表达升高,并参与卵巢上皮性癌的发生发展过程,血清KLK8检测可指导卵巢癌的早期诊断。Objective: To explore the expression of kallikrein gene 8 ( KLK8 ) and its significance in ovarian carcinoma, nethods:Immunohistochemistry was used to detect the expression of KLK8 protein in 93 epithelial ovarian tumor patients, including 20 cases of benign ovarian tumor, 11 cases of borderline ovarian tumor and 62 cases of ovarian carcinoma. Mean gray degree of the positive cell of staining section( A value) was measured. The double antibody sandwich enzyme-linked immunosorbent assay(ELISA) technique was applied to detect serum KLK8 protein concentration. The correlation between serum KLK8 concentration and tissue KLK8 protein in epithelial ovarian cancer patients were analyzed. Sensitivity and specificity of serum KLK8, CA125 in diagnosing ovarian carcinoma were compared. Results: ( 1 ) The positive expression rates of KLK8 protein in benign, borderline ovarian tumor and ovarian carcinoma were 25% ,27.3% and 66.1% respectively. The expression rate in ovarian carcinoma group was signficantly higher than that in benign ovarian tumor and borderline ovarian tumor groups ( P 〈 0.05 ). There was significant difference in different clinicopathological character of epithelial ovarian cancer. (2) The levels of serum KLK8 in different groups were 4.26±0.29,5.26±0.46,6. 59±0. 15μg/L respectively and there was significant difference among the three groups( P 〈 0.01 and P 〈0.05 ). (3)The A value of KLK8 protein expression in ovarian carcinoma tissue was 156.4±14.7 ,which was positively correlated with the serum concentration, their correlation coefficients was 0.608 (P 〈 0.001 ). (4) Sensitivity and specificity of serum KLK8 in diagnosing ovarian carcinoma was 61.3% and 77.4% respectively. Compared with CA125 ,there was no statistically difference (P 〉 0.05 ). Conclusion: The expression of KLK8 protein increases in patients with epithelial ovarian carcinoma. KLK8 is involved in the development of epithelial ovarian carcinoma and may be a molecular candidate
