奥沙利铂联合表阿霉素、5-FU/CF治疗晚期胃癌的临床观察

摘　　要：目的:观察奥沙利铂联合表阿霉素、5-FU/CF治疗晚期胃癌的疗效与安全性。方法:经组织病理学或细胞学证实的24例晚期胃癌患者,采用表柔比星50mg/m2静脉推注,总剂量分两天给予,d2、d3;奥沙利铂130mg/m2,静脉滴注3h,d1;亚叶酸钙200mg/m2,静脉滴注2h,qd×5;亚叶酸钙输注后,氟尿嘧啶600mg/m2,静脉滴注≥8h,qd×5;每3周重复,化疗2周期评价1次疗效。按WHO标准评价近期疗效和不良反应。结果:入组的24例晚期胃癌患者,其中进展期胃癌无法手术6例,术后复发转移18例。经治疗后CR2例(8.4%),PR8例(33.3%),SD8例(33.3%),PD6例(25%)。主要不良反应为骨髓抑制、黏膜炎、肢端麻木及消化道反应。全组患者无化疗相关死亡。结论:改良ECF方案治疗晚期胃癌疗效较好,不良反应轻,是安全有效的晚期胃癌治疗方案,可用于一线治疗,但其疗效、TTP、生存期等资料还需扩大样本进一步研究。Objective：To evaluate the efficacy and toxicity of epirubicin combined with oxaliplatin and 5 - FU/CF in the treatment of advanced gastric cancer. Methods ： Twenty four cases with advanced gastric cancer pathologically or cytologically proved received the treatment of epirubicin 50mg/m^2 iv at day2 and day3, oxaliplatin 130mg/m^2 ivgtt at dayl and 5 -fluorouracil 600mg,/m^2 dayl -day 5 （continuous injection exceed or be equal to 8 hours）, and previous calcium leucovorin 200mg/m^2 ivgtt at dayl - day5, repeated every 3 weeks. Efficacy was analyzed every 2 cycles. Results ：Twenty -four patients（6 patients with advanced gastric cancer who can＇t be operated, 18 patients with metastatic gastric cancer after surgical operation） were included into this study, 2 patients achieved CR （8.3%）, 8 PR （33.3%） ,8 SD（33.3% ） ,6 PD（25% ）. The total of 108 cycles of treatment were conducted in 24 patients,with grade 3, 4 toxicity of neutropeniain 16.7%, anemia 4.2%, thrombocytopenia 4.2%, nausea/vomiting 4.2%, mucositis 4.2%, and phlebitis 4.2%. There was no grade 4 toxicity except thrombocytopenia. There were no drug related heart dysfunction, arrhythmia, abnormality of alanine aminotransferase or creatinine. No treatment related death occurred. Conclusion： ECF modified regimen was well tolerated and effective for the treatment of patients with advanced gastric cancer. It could be used as the first line of treatment, but its efficacy, TTP, and overall survival still need to be further investigated.
