摘　　要：背景与目的探讨18F-FDG符合线路显像在妇科恶性肿瘤复发中的应用价值。方法选择89例妇科恶性肿瘤患者,分为两组,第1组18例,治疗后常规行SPECT18F-FDG检查排除有无复发肿瘤;第2组71例,临床怀疑肿瘤复发,行SPECT18F-FDG检查明确诊断。结果第1组18例患者中,15例初次18F-FDG显像均阴性,其中1例2年后18F-FDG显像阳性,肠镜及手术证实为肠原发癌;3例阳性者考虑术后改变,随访至今一年以上无复发证据。第2组71例患者中,60例18F-FDG显像阳性患者,其中15例行剖腹探查术证实肿瘤复发;45例患者给予化疗或放疗后,有26例复发病灶缩小,15例治疗无效死亡;4例随访3年病灶无明显变化。11例18F-FDG显像阴性患者,其中3例第2次18F-FDG显像阳性,经手术证实为复发,1例3次18F-FDG显像阴性,后剖腹探查明确为肠转移;余7例患者随访至今无复发迹象。结论对临床怀疑复发的妇科恶性肿瘤患者,18F-FDG显像检查能尽早发现复发病灶,及时给于治疗,提高其生存率,但要注意鉴别生理性摄取、疤痕及炎症。Background and objective To evaluate the effect of SPECT ^18F-FDG in diagnosing recurrent gynecologic carcinoma. Methods 89 patients with gynecologic carcinoma were included in this study. Group A： 18 patients underwent SPECT ^18F-FDG after treatment to exclude recurrence, Group B： 71 patients who were suspicious of recurrence underwent SPECT ^18F-FDG. Results In group A： 15 cases were with negative results, 1 case was found positive result two years later proved to be original tumor of intestine; 3case with positive result was diagnosed post-treatment changes. Group B： In 60 cases with positive results, 15 case were proved to recurrent by operation, 26 cases were found reducing recurrent tumor for chemotherapy/radiotherapy, 15 cases died of carcinoma, 4 case was proved tumor-free by 3-years follow up. In 1 lcases with negative results, 3 patients with second ^18F-FDG tomograhy were positive ,and proved to be recurret by surgery. 1 case with negative results for three times was proved to be intestine metastasis by operation, 7cases were followed up till now with no evidence of recurrence. Conclusion SPECT ^18F-FDG was helpful of diagnosing and treating recurrent carcinoma earlier, increasing survival rate. Recurrence should be differentiate from inflammation, scar and physiological uptake.
