凋亡抑制基因Survivin在宫颈癌中的表达及与HPV16／18的相关性研究

摘　　要：目的研究凋亡抑制基因Survivin在宫颈癌中的表达及与HPV16／18的关系，以探讨宫颈癌的发生机制，指导诊断和预后判断。方法采用原位杂交法检测20例正常宫颈组织、74例宫颈上皮内瘤变（CIN）和81例宫颈癌中SurvivinmRNA及HPV16／18DNA的表达。结果Survivin mRNA的阳性率在正常宫颈组织、CIN、宫颈癌中逐渐升高，且随着临床分期的升高、组织分化的恶化而更为升高，其阳性率与淋巴结转移相关，与病理类型及肿块类型无关。HPV16／18在正常宫颈组织、CIN、宫颈癌中的阳性率也逐渐升高，差异有统计学意义。SurvivinmRNA表达与HPV16／18感染呈正相关。结论SurvivinmRNA在宫颈癌中有异常表达。Survivin的异常表达和HPV16／18感染有关，它可能在HPV的协同作用下参与宫颈癌的发生和发展，两者联合检测可望为宫颈癌早期诊断和预后判断提供切实可行的指标。Objective To investigate the expression of anti-apoptosis gene survivin in cervical carcinoma and its correlation with HPV16/18. Methods The expression of survivin mRNA and HPV16/18 DNA were detected by hybridization in situ in 20 cases of normal cervical tissues and 74 cases of cervical intraepithelial neoplasia （CIN） and 81 cases of cervical carcinoma. Results The positive rate of survivin mRNA was rising with the progress from normal cervical tissues, CIN to cervical carcinoma. And it was related with clinical stage, differentiation, and lymph node metastasis in cervical carcinoma. There was no correlation between expression of survivin mRNA and pathological type or tumor type of cervical carcinoma. The infection rate of HPV16/18 was increased with the progress from normal cervical tissue, CIN to cervical carcinoma. The expression of survivin mRNA was positively correlated with the HPV 16/18 infection in cervical carcinoma. Conclusion The results suggest that survivin gene may be involved in the carcinogenesis of cervical cancer with HPV16/18; and the expression of surviving mRNA can be used as an indicator for diagnosis and prognosis of cervical carcinoma.
