摘 要:目的:观察乳腺癌患者原发灶的超声诊断图像和彩色多普勒特点,分析淋巴结转移(LNM)的相关因素,以获得术前诊断乳腺癌患者腋下LNM的指标。方法:超声观察133例乳腺癌患者的原发灶。观察原发灶的位置、最大径、数量、内部钙化灶、肿瘤内部的血流丰富程度分级(0~Ⅲ级)、阻力指数。以患者腋LN清扫的病理结果作为金标准,对与LNM相关的影像及临床因素进行分析。结果:133例乳腺癌患者的超声观察指标单因素分析及多因素分析(Logistic回归)结果显示,原发灶的最大径、数量、血流丰富度、有无钙化灶,对腋LNM有影响,差异有统计学意义(P<0.05)。多因素分析表明与腋LNM有关的影响因素为:原发灶的最大径、数量、血流丰富度。结论:对乳腺癌患者进行超声扫查,综合考虑原发灶的最大径、数量、血流丰富度,将有助于提高超声诊断腋LNM的正确率,较为准确地判断腋LN的状态。Objective: Obtain the index of axillary lymph node metastasis features and the related factors in breast cancer patients. Methods: The by analysis of the ultrasonographic ultrasonogrphic features of the primary tumor were observed in 133 patients. The primary tumor size, amount, micro-calcification, abundance of blood supply in the primary tumor (0-Ⅲ stage) were analyzed. A analysis of INM related ultrasonography and clinical factors was also performed. Results. Univariate analysis and multivariate analysis were performed to indicate that the primacy tumor size, amount, abundance of blood supply and micro-calcification have influences on the axillary lymph node metastasis in breast cancer, the differences were sta tistically significant (P 〈0.05). The primary tumor size, amount, abundance of blood supply were proved to be related to axillary lymph node metastasis by multiple logistic analysis. Conclusion: Ultrasonography is helpful in staging the axillary lymph node in breast cancer patient by observe. The primary tumor size,amount, and abundance of blood supply.
