脑梗死后相关神经纤维继发性损害的MRI研究

摘  要:目的:应用磁共振弥散张量成像(DTI)技术,动态观察皮质下脑梗死后神经纤维的顺行性、逆行性变性的过程。方法:选取10例单侧内囊为唯一梗死病灶的患者,分别在发病的第1周、6周、12周进行DTI检测,得到半卵圆中心、放射冠、内囊、大脑脚、脑桥等部位的部分各向异性图(FA)并测量其值。结果:在梗死灶同侧近端纤维束行程上和远端锥体束经路上都可出现清晰可见的信号减弱区,从发病的第1周至12周FA值明显降低。结论:皮质下脑梗死除了原发部位的损伤,还可以出现其近端、远端纤维的继发性变性改变。Objective To investigate the time course of anterograde and retrograde degeneration in neural fiber through diffusion tensor imaging (DTI). Methods Ten patients with a recent unilateral internal capsule infarct were evaluated with DTI at the first week (W1), the sixth (W6) and twelfth week (W12) respectively. Fractional anisotropy (FA) were measured at centrum semiovale, corona radiate, internal capsule, cerebral peduncle, and pons levels. Results Compared with the controls, the FA values of infarct side in the ipsilateral proximal fiber and distal portion of pyramidal was significantly decreased at W1, W6 and W12. Conclusion Subcortical cerebral infarction may result in anterograde degeration and retrograde degeneration in fiber tract through the primary lesion.
