乳腺癌组织CD44表达及其临床意义的探讨

摘　　要：目的:探讨CD44在乳腺癌组织中的表达及意义。方法:SP免疫组织化学方法检测101例乳腺癌组织CD44表达,χ2检验分析CD44与乳腺癌生物学特征的相关性,Log-rank检验分析CD44与乳腺癌预后的关系。结果:CD44在乳腺癌组织中过表达,阳性率为68.3%(69/101);CD44过表达阳性组与对照组的乳腺癌淋巴结转移差异有统计学意义,χ2=8.56,P=0.003 4;与肿瘤大小、病理组织学分级、ER、PR和HER-2/neu差异无统计学意义。CD44过表达阳性组与对照组乳腺癌患者36个月的无病生存率分别为75.2%、93.3%,CD44过表达阳性组明显低于对照组,χ2=4.905,P=0.027;CD44过表达阳性组与对照组乳腺癌患者总生存率分别为81.2%、97.8%,CD44过表达阳性组明显低于对照组,χ2=4.714,P=0.030。结论:CD44在乳腺癌组织中过表达,CD44过表达与乳腺癌淋巴结转移有关,CD44过表达是乳腺癌预后差的一个重要指标。OBJECTIVE：To investigate the expression of CD44 in breast cancer tissues and its significance. METHODS： The ex- pression of CD44 in breast cancer tissues was detected in 101 pa dents by the immunohistochemical SP method. The relationship be- tween the expressions of CD44 and biological characteristic of breast cancer was compared by x2 test. The relationship between CD44 and prognosis for breast cancer was evaluated by Log rank single- factor survival analysis. RESULTS： CD44 was over expressed in breast cancer tissues, and the percentage was 68. 3% （69/101）. The significant difference in lymph nodes metastasis of breast canc- er was found between the group of CD44 over-expression and the control group （X2=8.56,P=0.003 4）, but the tumor size, and histological grade, ER, PR and HER 2/neu did not exist statistical significance. The 3 year disease-free survival rate of high expres- sion group was much lower than the low expression group （75.2 vs 93.3%, 2=4. 905,P=0. 027）. The overal survival rate of the high expression group was much lower than that of the low expres- sion group （81.2% vs 97.8%, ？f2=4.714,P=0.030）. CONCLU- SIONS： CD44 is over expressed in breast cancer. The expression of CD44 may be closely related to lymph metastasis. Over-expressed CD44 may be an important marker for poor prognosis of breast cancer.
