摘　　要：目的：探讨血清甘氨酰脯氨酸二肽氨基肽酶（ＧＰＤＡ）活性测定对胃癌的诊断价值及方法学改良。方法：速率法（酶法）检测良恶性胃病患者的ＧＰＤＡ活性。结果：胃癌（无转移）组２５．３３±６．８５Ｕ／Ｌ；胃癌（肝转移）组１５６．９２±１４．６５Ｕ／Ｌ；胃溃疡组７６．８１±１１．５２Ｕ／Ｌ；正常对照组７８．１９±１０．９７Ｕ／Ｌ。胃癌无转移的ＧＰＤＡ活性显著低于正常对照组及胃溃疡组（Ｐ＜０．０１），但胃癌伴肝转移者却显著高于其它各组（Ｐ＜０．０１）。本法线性良好，批内变异系数（ＣＶ）分别为１．９８％、１．２６％、２．１８％；批间ＣＶ分别为２．３５％、２．０９％、３．１８％；经对比试验相关性良好。结论：本法速度快，精度高。ＧＰＤＡ可作为早期胃癌诊断的指标之一，并能对胃癌的肝转移进行有效的监控。Objective:To study the improvement in method of testing activities of glycylproline dipeptidylaminopeptidase(GPDA) and its value in diagnosis of gastric carcinoma.Methods:Activities of GPDA were determined by kinetics monitor.Results:The activities of GPDA in gastric carcinoma with no metastasis,in gastric carcinoma with hepatic metastasis,in gastric ulcer,in healthy individuals were 25.33±6.85,156.92±14.65,76.81±11.52 and 78.19±10.97 U/L respectively.The activities of GPDA in gastric carcinoma with no metastasis were significantly lower than that in gastric ulcer and normal individuals( P <0.01).However the activities of GPDA in gastric carcinoma with hepatic metastasis were higher than that in other groups.Conclusions:GPDA can be a marker for diagnosis of early gastric carcinoma and for monitoring hepatic metastasis in gastric carcinoma.
