肿瘤标志物对胃癌诊断应用价值的比较

摘　　要：目的:探讨并比较血清肿瘤标志物AFP、CEA、CA125、CA19-9、CA72-4及细胞角蛋白21-1对胃癌诊断的临床应用价值。方法:检测71例胃癌患者及31例慢性胃炎患者血清肿瘤标志物水平,计算并比较各指标敏感性、特异性、诊断准确性及ROC曲线下面积,研究各肿瘤标志物与胃癌淋巴结转移及浸润深度之间的关系。结果:胃癌组CA125、CA19-9、CA72-4水平明显高于慢性胃炎组(P<0.05)。CA72-4、CA125的诊断准确性和ROC曲线下面积优于其它指标。CA72-4、CA125、CA19-9水平在胃癌有淋巴结转移组高于无淋巴结转移组(P<0.05)。胃癌浸润至浆膜层组的CA72-4、CA125、CA19-9水平高于胃癌局限于粘膜及粘膜下层组(P<0.05)。结论:CA72-4、CA125、CA19-9对胃癌的诊断的临床应用价值优于AFP、CEA及细胞角蛋白21-1。Objective： To investigate the clinical value of serum tumor markers in gastric carcinoma. Methods： Serum tumor marker levels of AFP, CEA, CA125, CA19-9, CA72-4 and cyrfa21-1 were measured in 71 gastric carcinoma and 31 chronic gastritis. The sensitivity, specificity, diagnosis accuracy and ROC curve was calculated. All serum tumor markers were analyzed with lymph metastasis and depth infiltration in gastric carcinoma. Results：Levels of CA125, CA19-9 and CA72-4 in gas- tric carcinoma group were higher than those in chronic gastritis group （P〈0.05）. The diagnosis accuracy of eyrfa21-1, CA72- 4 and CA125 were better than other tumor markers. Levels of CA125, CA19-9 and CA72-4 in gastric carcinoma lymph metastasis group were increased when compared with no lymph metastasis group （P〈0.05）. The levels of CA125 and CA19-9 in serosa group were higher than mucosa and submucosa group. Conclusion：The clinical value of serum CA72-4, CA125 and CA19- 9 in diagnosis of gastric carcinoma is greater than AFP, CEA and cyrfa21-1.
