摘  要:目的:比较普通结肠镜与色素放大内镜(FICE系统)对结肠粘膜肿瘤样病变和非肿瘤病变的诊断和鉴别诊断价值。方法:对2008年3月-2008年9月择期行结肠镜检查的患者分别进行普通结肠镜检查、FICE系统检查及病理组织学诊断;以病理组织学诊断为"金标准",比较普通结肠镜与FICE系统诊断为肿瘤性病变和非肿瘤性的病变的灵敏度、特异度以及符合率。结果:共纳入30例患者,发现病变48个。与病理组织学诊断比较,普通结肠镜诊断结肠粘膜诊断肿瘤性和非肿瘤性病变的灵敏度、特异度和符合率依次为66.7%、87.5%和83.3%;FICE系统的灵敏度、特异度和符合率依次为83.3%、91.7%和90.0%,显著高于普通结肠内镜检查结果(P<0.05)。结论:对结肠粘膜病变FICE系统较传统结肠镜检能更好的鉴别结肠肿瘤样病变与非肿瘤样病变。Objective:To evaluate the clinical value of magnifying chromoscopy (FICE system) in combination with Kudo criteria in detection and diagnosis of neoplastic and nonneoplastic colonic lesions. Methods:After conventional electronic colonoscopic diagnosis of 30 colorectal lesions, magnifying observation by chromoscopy (FICE system) were made with Kudo criteria, and biopsy specimen and resected samples were sent for pathologic examinations. Results: A total of 48 lesions were detected in 30 patients. The sensitivity, specificity, and diagnosis accuracy of ordinary colonoscopy and magnifving colonoseonv (FICE system) compared with histological diagnosis of neoplastic and nonneoplastic polyps were 66.7 %, 87.5 % and 83.3 %, respectively;83.3%, 91.7% and 90.0%,respectively. Conclusion: Magnifying chromoscopy (FICE system) is valuable in detection of neoplastic and nonneoplastic colonic mucosal lesions, and has efficacy in judging the nature of lesions with Kudo criteria.
