摘 要:目的:研究胃癌组织中白细胞介素-8(IL-8)mRNA、单核细胞趋化蛋白-1(MCP-1)mRNA和巨噬细胞炎性蛋白-1α(MIP-1α)mRNA表达水平及肿瘤相关巨噬细胞(TAM)、肥大细胞(MC)计数,探讨它们之间的相互关系及临床病理意义。方法:51例胃癌常规制作石蜡包埋切片,应用原位杂交方法检测胃癌组织IL-8 mRNA、MCP-1 mRNA和MIP-1α mRNA的表达水平,并应用免疫组化方法测定胃癌组织TAM和MC计数,比较不同临床、病理特征胃癌患者3种趋化因子的表达及TAM、MC计数,并比较IL-8 mRNA、MCP-1 mRNA、MIP-1α mRNA不同表达者TAM和MC计数。结果:高、中分化及Ⅰ+Ⅱ期、无淋巴结转移、浸润深度T1+T2患者IL-8 mRNA、MCP-1 mRNA、MIP-1α mRNA阳性表达率及TAM和MC计数明显低于低、未分化及Ⅲ+Ⅳ期、淋巴结转移、浸润深度T3+T4者(P<0.01);IL-8 mRNA、MCP-1 mRNA和MIP-1α mRNA阳性表达病例TAM、MC计数均明显高于阴性病例(P<0.01)。结论:IL-8、MCP-1和MIP-1α3种化学趋化因子mRNA及TAM、MC计数可作为反映胃癌进展、生物学行为和预后的重要标志物,3种化学趋化因子mRNA均能促进癌基质中TAM、MC浸润和迁移。Objective: To determine relations of expressions of IL-8 mRNA, MCP-1 mRNA, MIP-1α mRNA and the counts of TAM and MC in gastric carcinoma and clinical pathological significance. Methods: Expression levels of IL-8mRNA, MCP-1 mRNA and MIP-1α mRNA were determined using in situ hybridization in paraffin-embedded sections of s gastric cancer were determin amples of 5! patients with gastric cancer, and TAM and MC counts of the samples of ed by immunohistochemistry. Expression levels of IL-8mRNA, MCP-1mRNA and MIP-1α mRNA, and counts of TAM and MC were compared according to different clinical and pathological features of gastric cancer, and relations between expression levels of IL-8mRNA, MCP-1 mRNA and MIP-1α mRNA and different counts of TAM and MC were studied. Results: Positive expression rates of IL-8mRNA, MCP-1mRNA, MIP-1α mRNA and counts of MC and TAM in patients of high and medium degree differentiation, clinical stage Ⅰ +Ⅱ , no lymph node metastasis, and infiltration of T1 + T2 were significantly lower than those low degree differentiation and undifferentiation, clinical stage Ⅲ+ Ⅳ, lymph node metastasis, and infiltration of 23 + T4 ( P 〈 0.01 ) ; counts of TAM and MC in cases of positive expression of IL-8mRNA, MCP-1 mRNA and MIP-1α mRNA were significantly higher than those in negative cases (P 〈 0.01 ). Conclusion: Chemokines of IL-8mRNA, MCP-1mRNA and MIP-1α mRNA can promote infiltration and migration of TAM and MC in gastric carcinoma, which and counts of TAM and MC may be important markers of the biological behavior, progress and prognosis of gastric carcinoma.
