脑白质疏松症的核磁共振成像及临床探讨

摘　　要：为探讨脑白质疏松症核磁共振成像和临床特征以及与脑梗塞的关系。本文收集１９９５～１９９７年脑白质疏松症５３例进行探讨分析。发现智力下降、肌张力升高、肌力下降、脑萎缩在脑白质疏松伴脑梗塞患者较多见，而在单纯脑白质疏松患者却无明显神经局灶表现，仅与智力下降及中央性脑萎缩有关。本病的起病方式及病程有规律性，提示脑白质疏松与脑栓塞有密切关系。Objective:To approach the MRI and clinical features of leukoaraiosis Methods:Age matched groups of patients with a MRI finding of pure leuki-araiosis(n=15),leuki-araiosis with brain infarction(s)(n=38),pure brain infarction and a control group with normal MRI finding(n=34)were formed (mean ages:653;647;630;635 years;NS respectively)Results: Dementia,cortical and central brain atrophy on MRI,gait disorder,focal neurological symptoms and signs were found to be more commonly present in leuko-araiosis with brain infarction(s) than in prue infarction(s) and in controls,whereas pure leuko-araiosis is related with dementia,The occurrences of heart failure were higher in the leukoaraiosis with infarction(s) than in the controlsleuko-araiosis was also found to be related to a less sudden onset of symptoms than pure brain infarction(s)Conclusions:leuko-araiosis on MRI in these elderly patients seem to be a vascular disorder aetiologically different from brain infarction,with clinical manifestations of subtle onset and no prominent focal neurological signs or symptoms \ \
