摘 要:目的分析准分子激光原位角膜磨镶术(LASIK)术后医源性高阶像差对成像质量及视功能的影响。方法利用自适应光学系统,对LASIK术后有视觉症状的12例患者进行像差矫正。在暗室内,采用5g/L新福林滴眼液右眼散瞳后,实施6mm瞳孔下像差矫正,测定100%对比度视力,计算不同像差矫正策略下的调制传递函数曲线和点扩散函数。结果与静态结果相比,受试者眼调制传递函数曲线和点扩散函数逐步提高,100%对比度视力也表现出类似的趋势。在矫正低阶像差的基础上,进一步矫正三阶像差使视力提高0.079LogMAR单位(P=0.016)。结论矫正医源性高阶像差可获得大瞳孔或暗环境下光学质量和高对比度视力的逐步提高,三阶像差对视觉质量的影响较大。Objective Present study was to analyse the effects of post-LASIK iatrogenic high order aberrations on image quality and visual function. Methods Adaptive optics was used for the correction of the aberrations in 12 right eyes with visual symptoms after LASIK. In a dark room,the pupils of the right eyes were dilated to 6 mm with 5 g/L phenylephedrine. Different aberration correction patterns were performed upon 6 mm pupil to measure the 100% contrast visual acuity. Modulation transfer function curve and point spread function of different aberration correction patterns were calculated based on the root mean square (RMS) of residual aberration. Results Compared with the static result,modulation transfer function curve and point spread function were gradually improved under the aberration correction patterns. Similar trend was found in the 100% contrast visual acuity in the subjects. After correction of the low order aberrations, the visual acuity increased by 0. 031 LogMAR, showing a significant difference in comparison with stationary status (P = 0. 273 ). Based on the correction of low order aberrations, further correction of the 3rd order aberration increased the visual acuity by 0. 079 LogMAR unit (P : 0. 016). However, the visual acuity declined by 0. 018 LogMAR in correction group of the 4th order aberration compared with correction group of the 3rd order aberration(P = 0. 473 ). After further correction of all aberration, visual acuity enhanced by 0. 047 LogMAR ( P = 0. 076). Conclusion Correction of iatrogenic high order aberrations could improve the optical quality and high contrast visual acuity under the 6 mm pupil size and scotopic condition. The 3rd order aberration influent the visual quality obviously.
