U型硬膜外导管联合Y型硅胶管植入术治疗全程泪道阻塞

摘　　要：目的:在泪道插管术中发现U型硬膜外导管插管术只能治疗泪小管阻塞,而Y型硅胶管植入术只能治疗鼻泪管阻塞,临床中观察到U型硬膜外导管拔出后鼻泪管阻塞仍存在,Y型硅胶管植入术对泪小管阻塞无作用,所以我们改进方法观察U型硬膜外导管联合Y型硅胶管植入术治疗全程泪道阻塞的临床疗效,评价其应用价值。方法:对26例(35眼)泪道阻塞患者行U型硬膜外导管联合Y型硅胶管植入术,此为治疗组;另外对28例(34眼)泪道阻塞患者行单纯U型硬膜外导管植入术或硅胶导管植入术,此为观察组;对2组治疗效果进行比较。结果:治疗组所有患者术后1个月、3个月通畅率均为91.4%,术后半年及1年有6例(8眼)发生不同程度的堵塞,通畅率为77.1%和71.4%。观察组术后第1个月通畅率为73.5%,术后第3个月通畅率为64.7%,术后半年通畅率为50%,术后1年通畅率为47.1%,两组患者疗效比较,差异具有统计学意义(P<0.05)。结论:U型硬膜外导管联合Y型硅胶管植入术对治疗全程泪道阻塞效果确切,复发率低,值得临床推广。Objective：We had found that U-epidural catheter intubation only treat lacrimal duct obstruction in the Lacrimal intubation, and the Y-silicagel tube implantation only treat nasolacrimal duct obstruction, we observed that Nasolacrimal duct obstruction still after the pull-out Epidural catheter In clinical, it was of no use that Y-silicagel tube implantation treat lacrimal duct obstruction, so we observed clinical efficacy of U-epidural catheter combined Y-silicagel tube implantation treating the whole duct obstruction by improving methods, estimate its application value. Methods： Use U-epidural catheter combined Y-silicagel tube implantation treating 26 cases （35） in patients with duct obstruction, this was the treatment group; in addition, using simple U-epidural catheter implantation or silicone catheter implantation treating 28 cases （34） in patients with duct obstruction, this was the observation group; compare treatment effect of two. Results： 6 cases （8） occur in varying degrees of plug after six months and one year, patency rate was 77.1%, 3 months patency rate in the observation group after a month was 73.5%, other cases occur in a certain degree of blockage, even completely block, patency rate after three months was 64.7%, patency rate after six months was 50%, patency rate ＇after one year was 47.1%, efficacy of the two groups patients had significant differences （P〈0.05）. Conclusion：It is valued that U-epidural catheter combined Y-sil icagel tube implantation treat the whole duct obstruction, relapse rate low, worthy to be popularized.
