牙龈部扁平苔癣患者伴发牙周病80例临床分析

摘　　要：目的观察发生在牙龈部的扁平苔癣伴发牙周病的情况及病损程度，讨论治疗牙龈部扁平苔癣伴发牙周病时的思路。方法对360例经临床和组织病理学确诊为口腔扁平苔癣（OLP）的患者的发病部位及是否伴发牙周病的情况进行分析。结果360例OPL患者中80例为口腔其他部位同时伴发牙龈部位的扁平苔癣患者，其中74例（92．5％）的牙龈部的扁平苔癣伴发牙周病，不伴有牙龈部位扁平苔藓的OLP患者中并发牙周病者41例（14．6％），二者牙周病的伴发率比较差异有统计学意义（χ^2=33．669，P〈0．05）；口腔牙龈部扁平苔癣病变的严重程度常与口腔中其他好发部位的扁平苔癣严重程度相同（uc=1．920，P〉0．05）。结论牙龈部的扁平苔癣常伴发牙周病，可能都有相同的致病因素，同时治疗牙周病和口腔扁平苔癣可能会同时提高这两种病的治疗疗效。Objective To observe the patients with gingival involvement of oral lichen planus often accompanies periodontal disease; to explore the identity between simple periodontal disease and gingival involvement of oral lichen planus with periodontal disease; to discuss the idea of curing gingival involvement of oral lichen planus with periodontal disease. Methods The paper have analyzed synthetically the diseased regian and the information of complicated periodontal disease of 360 cases clinically and histopathologically assessed patients affected by OLP. Results It was found that 92.5 % of patients of gingival involvement of oral lichen planus accompanies periodontal disease （ χ^2 = 33. 669, P 〈 0.05 ）. The serious degree of lesions is same in gingival involvement of oral lichen planus and other location involvement of oral lichen planus（ uc = 1. 920,P 〉 0.05）. Conclusion The most patients of gingival involvement of oral lichen planus accompanies periodontal disease, possibly all has the same pathogenesis factor, simultaneously treatment periodontal disease and oral lichen planus possibly can simultaneously enhance the treatment curative effect of these two kind of sickness.
