摘 要:目的:研究无嘌呤/无嘧啶内切核酸酶(APE)和乏氧诱导因子1α(HIF-1α)表达与鼻咽癌生物学行为的关系,探讨其在鼻咽癌诊断和治疗中的价值。方法:应用免疫组化技术,检测47例鼻咽癌和20例鼻咽慢性炎性组织中APE和HIF-1α的表达,并对10例鼻咽癌患者放疗前和放疗期间组织中APE和HIF-1α表达水平进行动态检测。结果:鼻咽癌组织中APE的表达高于鼻咽慢性炎性组织,但其差异无统计学意义;APE表达与性别、年龄、T分期、有无颈淋巴结转移和临床分期均无关。鼻咽癌组织中HIF-1α的表达显著高于鼻咽慢性炎性组织;HIF-1α表达与T分期、有无颈淋巴结转移和临床分期相关,但与性别、年龄无关;放疗后获得肿瘤局部控制的患者,其放疗期间HIF-1α表达水平显著低于放疗前。鼻咽癌中APE和HIF-1α的表达未见相关性。结论:APE和HIF-1α在鼻咽癌中均呈过表达,提示两者均可能涉及鼻咽癌的发生、发展过程。HIF-1α表达水平的检测对鼻咽癌的诊断、分期及疗效判断有重要价值。Objective: To investigate the correlation of the expression of Apurinic/apyrimidinic endonuclase (APE) and Hypoxia-inducible Factor la (HIF-1α) with the clinical biological behavior of nasopharyngea carcinoma (NPC), and to explore the value of detecting these two indices in the diagnosis and treatment of NPC. Methods: The expression of APE and HIF-1α was examined with immunohistochemistry in 47 samples of NPC and 20 samples of chronic nasopharynxitis. The dynamic variations in the expression levels of APE and HIF-1α before radiotherapy (pre-RT) and during radiotherapy (dudng-RT) were detected in 10 cases of NPC patients. Results: The expression of APE in NPC was not significantly different from that in chronic nasopharynxitis. There were no correlations between APE expression and gender, age, T-stage, nodal involvement and clinical stage. HIF-1α expression in NPC was significantly higher than that in chronic nasopharynxitis. There were significant correlations between HIF-1α expression and T-stage, nodal involvement and clinical stage. No correlation was found between HIF-1α expression and gender and age. Expression levels of HIF-1α in patients during RT treatment was significantly decreased compared with that before RT treatment. There was no significant correlation between APE expression and HIF-1α expression in NPC. Conclusion: Overex- pression of APE and HIF-1α was common in NPC, and may be associated with the carcinogenesis and development of NPC. Detection of HIF-1α expression level is of great value in diagnosing and staging NPC and is helpful for the evaluation of therapeutic effect of treatment for NPC.
