康网灵对视网膜脱离复位术后残留视网膜下液的临床研究

摘　　要：目的观察中药康网灵汤剂对波及黄斑部的视网膜脱离患者复位术后黄斑部残留视网膜下液的影响。方法将2004年11月至2006年9月,脱离波及黄斑部的孔源性视网膜脱离,成功行视网膜复位术后经光学相干断层扫描(optical coherence tomography,OCT)证实黄斑部残留视网膜下液的患者60例60眼,随机分为2组,对照组常规处理,中药组在此基础上加服中药康网灵汤剂,3次/日,30天为1个疗程,治疗6个疗程。治疗前后行OCT、多焦视网膜电图(mERG)及视力检查。观察指标:治疗前和治疗后1、3、6个月黄斑部OCT、mERG的P1波振幅密度值(nV/deg2)及视力。数据结果均应用SPSS13.0软件进行统计学处理。结果治疗前两组在年龄、病程、视力、黄斑部残留视网膜下液量及P1波振幅密度值上差异均无统计学意义(P>0.05);治疗后中药组视力提高的眼数、黄斑部视网膜厚度及残留视网膜下液量减少,明显好于对照组;黄斑部mERG1、2环P1波振幅密度值在1、3个月均明显高于对照组(P<0.05),两组间差异有统计学意义。3、4、5环在3、6个月与对照组差异无统计学意义(P>0.05)。结论中药康网灵汤剂能促进残留视网膜下液吸收,改善视功能。OBJECTIVE To observe the effect of Kangwangling Decoction on macular residual subretinal fluid （SRF） after successful rhegmatogenous retinal detachment （RRD） surgery. METHODS Sixty cases with subretinal fluid （SRF） after successful rhegmatogenous retinal detachment surgery, confirmed by OCT, were randomly divided into two groups. Control group was treated with conventional method; traditional Chinese medicine group was treated with conventional method and Kangwangling Decoction, 3 times a day, thirty days as one course, all together 6 courses. Checked OCT,mERG and visual acuity before and after treatment. The data were analysed by SPSS13.0 statistical software. RESULTS There was no significant difference between the two groups in age, course, visual acuity,the quantity of residual SRF and the P1 amplitude density （nV/deg^2） （P〉0.05）. After treatment, compared with control group, the cases of improved visual acuity was higher, the quantity residual SRF was lower and the P1 amplitude density （nV/ deg^2） in 1, 2 rings of macular mERG at 1, 2, 3 months was higher significantly in traditional Chinese medicine group （P〈0.05）. There was no difference between the two groups in 3, 4 rings at 3, 6months （P〉 0.05）.CONCLUSIONS Kangwangling Decoction could help to decrease the residual subretinal fluid and to improve the visual function.
