大肠癌及癌旁粘膜细胞增殖活性的研究

摘　　要：目的:探讨大肠癌组织及癌旁粘膜细胞增殖水平。方法:采用免疫组织化学和嗜银染色技术对48例大肠癌组织及癌旁粘膜及10例正常粘膜中增殖核细胞抗原(PCNA)、核仁区嗜银染色计数(AgNORs)及癌基因c-myc、c-fos的表达进行检测。结果:大肠癌组织中PCNA标记指数(PCNA-LI),AgNORs计数及癌基因c-myc、c-fos阳性表达均显著高于癌旁3cm和6cm粘膜及正常粘膜(P<0.01),PCNA-LI在Dukes'C和D期显著高于Dukes'A期(P<0.05)。癌旁3cm粘膜细胞AgNORs计数显著高于癌旁6cm(P<0.01)及正常粘膜(P<0.05)。结论:大肠癌癌旁粘膜部分细胞增殖调节失控,反映癌旁粘膜是一种不稳定的癌前状态,癌旁粘膜细胞的高增殖活性可能是大肠癌术后复发的重要原因。Objective To study the cells proliferative level in colorectal carcinoma cells （CRC） and carcinoma adjacent mucosa （CAM） Methods The expression of PCNA and c-myc,c-fos in 48 cases of CRC, CAM and in 10 cases of normal mucosa was detected by immunohistochemistry techniques. AgNORs was determined with argyrophilia stain. Results The PCNA-LI, AgNORs count and the positive expression rate of c-myc,c-fos in CRC was higher than that in CAM and normal mucosa （P〈0.01） .The PCNA-LI in Dukes＇C and D stage was higher than that in Dukes＇A （P〈 0.05） . The AgNORs count in CMC 3cm was higher than CMC 6cm （P〈0.01） and normal mucosa （P〈0.05） . Conclusions The cells proliferative activity were abnormal in CAM. It indicates that CAM is an unstable premalignant change,which might have some correlation with the relapse of CRC.
