摘 要:目的探讨宫颈癌及宫颈上皮内瘤变(CIN)患者宫颈脱落细胞中人类染色体末端酶(hTERC)基因的表达变化及其诊断价值。方法选择接受宫颈病变筛查者共84例,治疗前获取宫颈脱落细胞,一部分行液基细胞学检查(LCT)、一部分行hTERC基因检测。结果病理诊断为CINⅠ、CINⅡ、CINⅢ和宫颈鳞状细胞癌(SCC)者分别为22、13、17、13例,其宫颈脱落细胞hTERC基因阳性率分别为36.36%、76.92%、82.35%和92.31%;LCT检测正常的9例及病理检查正常的10例中,宫颈脱落细胞hTERC基因均阴性;CIN与正常者的hTERC基因阳性率相比,P<0.05;高度宫颈病变(CINⅡ、CINⅢ、SCC)者与低度病变(CINⅠ)者hTERC基因阳性率相比,P<0.05。LCT联合hTERC基因检测筛查宫颈高度病变的检出率(88.37%)与单纯LCT的检出率(48.84%)相比,P<0.05。结论宫颈脱落细胞hTERC基因检测能够准确诊断宫颈病变,LCT联合hTERC基因检测能够提高宫颈病变的检出率。Objective To investigate the expression and diagnostic significance of the human telomerase (hTERC) gene in cervial cells of cervical cancer and cervical intraepithelial neoplasia(CIN). Methods 84 cytologic specimens were collected before examination and treatment. One haft was used for hTERC gene examination and the other for LCT. The patients with abnormal LCT results were asked for histology biopsy. Results According to histology biopsy, the specimens were divided into normal 10 cases, CINⅠ 22 eases, CIN Ⅱ 13 cases, CINⅢ 17 cases and squamous cervical cancer (SCC) 13 cases. None normal samples (9 specimens with normal LCT results and 10 specimens with normal histological results ) revealed positive expression of hTERC gene, while 36. 36% of the CINⅠ,76.92% of the CIN Ⅱ ,82.35% of the CINⅢ lesions and 92.31% of the SCC. Compared with normal cervix, hTERC gene positive expression in cervical lesion (CIN and SCC) was different significantly (P〈0.05), and there was a significantly difference(P 〈0.05) between lfigh grade lesion ( CIN Ⅱ , CIN Ⅲ, SCC )and low grade lesion( CIN Ⅰ ). The detection rate was significantly higher in the screening of cervical high grade lesion with LCT united hTERC genetic testing( 88.37% ) than that with LCT(48.84% ), P 〈 0.05. Conclution hTERC gene testing can accurately diagnose cervical lesions, LCT united hTERC gene testing can improve the detection rate of cervical lesions.
