孕妇HBV感染状态和HBV-DNA与母婴HBV传播关系

摘　　要：目的进一步探讨ＨＢＶ感染状态与母婴ＨＢＶ传播的关系。方法应用聚合酶链反应对１９２例不同ＨＢＶ感染状态孕妇和５９例乙型肝炎五项血清学标志阴性的孕妇做了母血、乳汁和脐血中ＨＢＶ－ＤＮＡ检测。并进行了对比研究。结果实验组（１９２例）孕妇血清、乳汁及脐血中ＨＢＶ－ＤＮＡ阳性率分别有５６．６％（７３／１９２）、２３．４％（４５／１９２）和３１．３％（６０／１９２），不同ＨＢＶ感染状态孕妇其对应脐血中ＨＢＶ－ＤＮＡ阳性率有较大差异（Ｐ＜０．０１），母血为“ＨＢｓＡｇ＋、ＨＢｅＡｇ＋、ＨＢｃＡｂ＋”和“ＨＢｓＡｇ＋、ＨＢｅＡｇ＋”两种感染状态，对应脐血中ＨＢＶ－ＤＮＡ阳性率最高，分别为７４．４％（２９／３９）和６５％（１３／２０）。对照组孕妇母血、乳汁和脐血ＨＢＶ－ＤＮＡ均未检出。母血ＨＢＶ－ＤＮＡ阳性的孕妇其脐血和乳汁中ＨＢＶ－ＤＮＡ阳性率分别为８２．３％（６０／７３）和６１．６％（４５／７３），而ＨＢＶ－ＤＮＡ阴性的孕妇其乳汁和脐血中均未检出ＨＢＶ－ＤＮＡ。结论母血中ＨＢＶ－ＤＮＡ阳性是造成母婴ＨＢＶ传播的主要因素。Objective To investigate the relationship between the replication status of HBV and motherinfact HBV transmission in pregnancy.Methods Serum markers of HBV infection (HBVM) were tested by ELISA in 251 pregnant women.HBVDNA was measured by polymerase chain reaction in serum,milk and cord blood from 192 pregnancies who were HBVM positive carrier and 59 normal control.Results HBVDNA positive rates in serum,milk and cord blood in the HBVM positive pregnancies were 56.6%(73/192) 23.4%(45/192)and 31.3%(60/192)respectively,it was significant difference in HBV infection status(P[WT5”BZ]<0.01).The positive rates of HBV DNA in cord blood from the pregnancies who were HBsAg(+), HBeAg(+), AtiHBc(+) and HBsAg(+), HBeAg(+) were 74.4%(29/39)and 65%(13/20).HBVDNA in samples from the normal controls were all negative. The rates of HBVDNA positive in cord blood and milk from pregnancies who were serum HBVDNA positive were 82.3%(60/73),61.6%(45/73),but it was negative in cord blood from the pregnancies who were serum HBVDNA negative.Conclusion The replication status of HBV and serum HBVDNA in pregnancies were associated with motherinfact HBV transmission.
