大脑凸面胶质瘤的手术治疗

摘　　要：目的探讨大脑凸面胶质瘤的手术治疗经验和治疗效果。方法对我院自1980年5月至2008年6月收治的91例胶质瘤患者的临床表现、影像学资料、手术切除肿瘤的方法及策略进行回顾性分析。结果本组常规开颅手术50例:肿瘤全手术切除率60%,术后死亡率8%,致残率8%,症状改善率60%,复发率46.4%,平均输血量800mL;显微外科手术41例:肿瘤全手术切除率75.6%,术后死亡率2.4%,致残率4.8%,症状改善率73.1%,复发率31.2%,平均输血量300mL,与常规手术组比较,3项指标均有显著差异(P<0.05)。结论显微外科手术能明显提高手术全切除率,减少死亡率及复发率,改善症状。To investigate the operative technique for the parasagittal and falcine meningiomas in the middle cortex areas and its curative effect. The clinical data of 81 patients wilh parasagittal and falcine meningiomas in the middle cortex areas,who underwent surgery in our hospital from April 1980 to April 2007,were analyzed retrospectively. A total removal rate of 60%,Pastoperative death rate2.4%,Mutilation rate 0, symptom intprovement rate 97.5%, average blood transfusion volume 1400 mL, Microsurgery 41 patients： a total removal rate of 87.8%, Postoperative death rate 7.5%,Mutilation rate 7.5%, symptom improvement rate 80%, average blood transfusion volume 600mL, with common operation to compare,3 index were significant deviation（P〈0.05）. Microsurgery can improve a total removal rate, postoperative death rate, Mutilation rate and symptom improvement.
