老年精神分裂患者探究性眼球运动异常的神经心理机制探讨

摘　　要：目的探讨精神分裂症探究性眼球运动轨迹异常与认知功能的关系。方法对35名住院老年精神分裂症患者及31名正常老年人进行眼动检查、威斯康星卡片分类测验（WCST）及成套注意测验。结果老年精神分裂症患者眼球活动的凝视点数（NEF）、反应性探索分（RSS）[分别为（26．88±4．79）分，（3．80±1．87）分]，低于对照组[分别为（30．41±3．90）分，（5．00±2．10）分]，差异具有显著性（t值分别为2．99和2．19，P〈0．01和P〈0．05）。成套注意测验和WCST中的多个指标与对照组比较差异有显著性。2组的NEF与注意测验中的节律测验得分、计算测验得分和WCST的正确应答数呈正相关（r=0．25～0．40），与WCST的完成第一个分类所需应答数呈负相关（r=-0．33，P〈0．05）。2组的RSS与WCST的完成第一个分类所需应答数呈负相关（r=-0．33，P〈0．05），与WCST的正确应答数呈正相关（r=0．41，P〈0．01）。结论额叶功能障碍．注意缺陷和执行功能受损可能是精神分裂症眼球活动异常的神经心理学机制。Objective To explore the relationship between exploratory eye movement and cognitive func- tion in geriatric schizophrenic patients. Methods 31 geriatric schizophrenic inpatients and 32 healthy aged people were involved in the study. All the subjects received the examination of exploratory eye movement and were assessed with Wisconsin Card Sorting Test and Attention Test Battery （ATB）. Results The number of Eye Fixation （NEF） and Responsive Search Score （RSS） [ （ 26.88 ± 4.79 ） , （ 3.80 ± 1.87 ） respectively ] of the geriatric schizophrenic patients were significantly lower than those [ （ 30.41 ± 3.90 ）, （ 5.00 ± 2. 10 ） respectively ] of the healthy aged people. The NEF of both groups were correlated positively with the scores of rhythm test and calculation test of ATB, positively with Rc of WCST （r = 0.25 ± 0.40）, negatively with Rf of WCST （r = - 0.33, P 〈 0.05 ）. The RSS of both groups were correlated negatively with Rf of WCST （r = - 0.33, P 〈 0.05 ）, positively with Rc of WCST （r = 0.41, P 〈 0.01 ）. Conclusion The attention deficit and poor ability of abstraction and generalization might be the psychological mechanism of exploratory eye movement in schizophrenic patients.
