齐拉西酮与氯氮平对精神分裂症疗效和执行功能影响的对照研究

摘　　要：目的评价齐拉西酮对精神分裂症患者的疗效和安全性，并研究其对执行功能的影响。方法选取84例精神分裂症患者，随机分为研究组42例（用齐拉西酮治疗）和对照组42例（用氯氮平治疗），疗程12周。在治疗前及治疗4，8，12周末分别采用阳性与阴性症状量表（PANSS）、副反应量表（TESS）进行疗效、不良反应评定。在治疗前及治疗12周末分别采用威斯康星卡片分类测验（WCST）评估执行功能。结果2组治疗前后PANSS评分均差异有显著性（P〈0．05或P〈0．01），治疗后组间比较差异无显著性俨〉0．05）。齐拉西酮不良反应明显低于氯氮平，差异具有显著性（P〈0．05或P〈0．01）。研究组总测验次数、持续错误数、随机错误数分值均低于对照组[（57．58±14．01）分，（67．51±20．99）分；（14．11±17．42）分，（18．05±5．49）分；（17．43±8．68）分，（24．34±7．61）分，P〈0．05或P〈0．01]。结论齐拉西酮对精神分裂症患者具有疗效好，不良反应少的特点，且执行功能优于氯氮平。Objective To evaluate the efficacy and safety and to study the effect on executive function of ziprasidone in treatment of schizophrenia. Methods A total of 84 schizophrenic inpatients were randomly divided into study group treated with ziprasidone（n = 42） and control group treated with clozapine （n = 42） for 12 weeks treatment. Positive and negative syndrome scale （PANSS） and treatment emergent symptoms scale （TESS） were used to evaluated the efficacy and the side effects before and in the end of 4th,8th,12th week. Wisconsin Card Sorting Test were used to evaluated executive function before and in the end of 12th week. Results After treatment, the scores of PANSS reduced significantly than those before treatment in two groups. （P〈0. 05 or P〈0. 01 ）. The side effects in study group were significantly fewer than those in control group. （P〈0.05 or P〈0. 01 ）. The total number,persistent error number and random error number in study group were significantly better than those in control group. （57.58 ±14.01 vs67.51±20.99;14.11 ±7.42 vs 18.05 ±5.49;17.43 ±8.68 vs24.34± 7.61, P 〈 0.05 or P 〈 0.01 ）. Conclusion Ziprasidone were effective and less side effects in the treatment of schizophrenia. Executive function in study group were significantly better than those in control group.
