一期切除吻合术在结肠癌并急性梗阻的临床应用

摘　　要：目的:探讨结肠癌合并肠梗阻一期切除吻合术的安全性及其临床应用。方法:2005年10月至2009年2月我院收治42例结肠癌合并肠梗阻患者,均行一期切除吻合术,术中进行有效肠腔减压和清洁灌洗。结果:所有手术均顺利完成;无吻合口漏等严重并发症发生,所有患者均痊愈出院。结论:结肠癌合并肠梗阻一期切除吻合术是安全的术式,但要掌握好适应证,加强围手术期处理,特别是术中应行有效肠减压和清洁灌洗。Objective ： To explore the safety and clinical application of one - stage resection and anastomosis in colon carcinoma with intestinal obstruction. Method： 42 cases with obstructing colon carcinoma were undergone one -stage resection and anastomosis in our hospital from Oct. 2005 to Feb. 2009. Bowel decompression and colonic lavage were carried out simultaneously in course of the operation. Result： All cases received one -stage resection and anastomosis successfully and there were no serious complications such as stomal leak. Conclusion： It is safe for the patients with obstructing colon carcinoma to undergo one - stage resection and anastomosis. But indications, preoperative management and aggressive bowel decompression and colonic lavage should be emphasized.
