摘  要:目的:探讨一氧化氮(NO)对肝硬化门静脉高压大鼠肠黏膜通透性的影响。方法:10只正常大鼠为对照组腹腔注射生理盐水2周,腹腔注射硫代乙酰胺(TAA)制成大鼠肝硬化模型30只,随机分为3组,分别给予腹腔注射生理盐水(NS组),左旋精氨酸(L-Arg)(L-Arg组)、左硝基精氨酸甲基酯(L-NAME)(L-NAME组)各10只,各2周。测定门静脉压力(PVP)、血浆NO、二胺氧化酶(DAO)、D-乳酸(D-LA)和内毒素(ETX)含量,观察回肠膜病理改变。结果:NS组大鼠肠黏膜损伤指数、PVP、NO、DAO、D-LA和ETX较对照组显著增高(P<0.05),L-Arg组上述指标较NS组显著升高(P<0.05),L-NAME组上述指标较NS组显著下降(P<0.05),各指标存在正相关关系。结论:门静脉压力升高是导致肝硬化大鼠肠道通透性升高的直接因素,而ETX和NO的升高维持了门静脉的高动力循环,降低NO水平可改善高动力循环,对肠黏膜通透性有一定的保护作用。Objective To research the effects of nitric oxide (NO) on intestinal mucosa permeability in rats with cirrhotic portal hypertension. Methods 10 rats received normal saline intraperitoneal injection for 2 weeks as control group. 30 rats, who received thioacetamide intraperitoneal injection as cirrhotic portal hypertension models, were randomly assigned to receive intraperitoneal injection of normal saline (NS group, n =10), L-arginine (L-Arg group, n = 10), and NG-nit-L-arginine methyl ester (L-NAME group, n = 10) for 2 weeks. Portal venous pressure (PVP) and plasma levels of NO, endotoxin, diamine oxidase, and D-lactic acid were measured, pathological changes of intestinal mucosa were observed. Results Intestinal damage index, PVP, NO, diamine oxidase, D-lactic acid, and endotoxin in NS group were significantly higher than those in control group and L-NAME group (P 〈 0.05), but were lower than those in L-Arg group (P 〈 0.05). There were significant correlations among intestinal damage index, PVP, NO, diamine oxidase, D-lactic acid, and endotoxin. Conclusions Elevation of portal pressure, which may be maintained by endotoxin and NO, plays an important role in the increase of intestinal permeability. Reducing the levels of NO is benefit to intestinal permeability.
