两种自体结膜移植术治疗原发翼状胬肉疗效比较

摘　　要：目的:比较带角膜缘的自体结膜移植与不带角膜缘的自体结膜移植对原发翼状胬肉术后效果的影响。方法:选取2007-03/2008-12在本院就诊的原发性翼状胬肉患者,年龄35～70岁。单眼手术,术者为同一人。分为不带角膜缘的自体结膜移植组和带角膜缘的自体结膜移植组。随访时间为1a。比较两种手术方法对复发率的影响。结果:去除失访的15例,不带角膜缘组42例中7例复发,带角膜缘组45例中1例复发。两组复发率的差异有统计学意义。即带角膜缘组的复发率较低,显著低于不带角膜缘组。两组的并发症均较少,且无严重并发症。结论:与不带角膜缘的自体结膜移植术相比,带角膜缘的自体结膜移植术技术难度与前者相似,但能更好的抑制术后复发。AIM： To compare the outcome of nomlimbal conjunctival autograft（NLCAU） and limbal conjunctival autograft （LCAU） for prevention of pterygial recurrence. METHODS： One hundred and two patients （102 eyes） with primary pterygium （from March 2007 to December 2008, age between 35-70 years, being operated unilaterally） were randomly divided into two groups： NLCAU group and LCAU group. Patients were followed up for one year. RESULTS： There were 7 recurrence cases（17%） in the NLCAU group（n=42） and 1 （2%） in the LCAU group （n= 45）, except 15 cases lost follow-up. There was statistically significant difference in the recurrence rate between the two groups （P= 0. 046）. Complications of the two groups decreased, and there were no severe compli-cations in both the two groups. CONCLUSION ： There was the same degree of technique difficulty both in limbal conjunctival autograft and non- limbal conjunctival autograft, but the former was better than the latter for preventing recurrence after surgery.
