胆管周围浸润型肝内胆管癌的CT诊断

摘　　要：目的探讨胆管周围浸润型肝内胆管癌(PIICC)的CT表现。资料与方法回顾性分析经临床病理证实的6例PIICC患者的CT平扫和多期动态增强扫描资料。结果PIICC表现为沿胆管纵轴生长的树枝状肿块,伴周围胆管扩张,动态增强扫描肿瘤呈持续性的延迟强化。肿瘤倾向沿胆管壁向肝门方向扩散,包绕门静脉分支并最终侵犯肝门部。结论PIICC的生长方式、形态特征和生物学行为不同于常见的肿块型肝内胆管癌,其CT表现具有一定的特征性。Objective To discuss the CT features of periductal infiltrating intrahepatic cholangiocareinoma （PIICC）. Materials and Methods Unenhaneed and muhi-phase dynamic contrast enhanced CT images of 6 patients with PIICC confirmed by pathology were reviewed. Results PIICC showed a branchlike mass that grew longitudinally along the bile duct with dilatation of the peripheral bile ducts. On dynamic contrast-enhanced CT imaging, the tumor showed persistent and delayed enhancement. PIICC tended to spread along the bile duct wall toward the hepatic portal, encircled the portal vein and invaded eventually the hepatic portal. Conclusion PIICC is different from the most common mass-forming intrahepatic eholangiocarcinoma in the growth pattern, morphologic appearance and biologic behavior. CT findings have some certain characteristics.
