摘 要:目的:探讨社会因素剖宫产对儿童体格与智力发育的影响。方法:采用前瞻性队列研究,确定社会因素剖宫产和顺产产妇队列(社会因素剖宫产组68例、顺产组65例),分别于出生时、18月龄、24月龄、30月龄、36月龄、42月龄时体格检查,24月龄时智力测验(采用Gesell评分)。结果:两队列均衡性较好(P>0.05),两组儿童在身高、体重、头围以及智力发育方面差异均无统计学意义。结论:社会因素剖宫产对42月龄以下的儿童体格发育未见显著性影响,社会因素剖宫产智力发育并不优于顺产,远期影响仍需扩大样本量以及继续随访研究。Objective: To explore the effect of elective cesarean section on physical and intellectual development in children. Methods: Using the prospective cohort study, a 133 newborn sample composed by 68 newborns delivered by cesarean section and 65 newborns by spontaneous delivery in a county of Chongqing was established from March to May, 2002. Physical development index such as length, head circumference and body weight were measured at birth and on the 18, 24, 30, 36 and 42 -month -old, respectively. Intellectual development was tested by Gesell mark at 2 years old. Results : The factors had equilibrium in two cohorts ( P 〉 0. 05 ) . There had no significant difference of weight, height or length, head circumference and Intellectual development between the two groups. Conclusion : There's no significant effect of cesarean section on physical growth of children below 4 years old. Intellectual development of the children delivered by cesarean section is no better than that of spontaneous delivery. Forward affect needs more samples and continual follow - up.
