摘 要:目的评价磁敏感加权成像在脑星形细胞瘤分级中的价值。方法经病理证实的22例脑星形细胞瘤患者,按世界卫生组织2000年规定的胶质瘤分级标准分为2组,高级别组(Ⅲ~Ⅳ级)13例,低级别组(Ⅰ~Ⅱ级)9例。使用GE3.0 TMR扫描仪进行常规序列(T1WI,T2WI,FLAIR,CE—T1WI)和SWI序列扫描。将SWI图像与常规序列比较,并与病理对照分析。SWI序列的参数如下:TR-35ms,TE=20ms,FA=15°,层厚2mm。分析SWI显示的肿瘤内静脉、出血及其代谢产物在2组脑星形细胞瘤间是否存在差异。结果SWI显示的出血及小静脉在2组星形细胞瘤中存在明显差异。SWI在显示肿瘤的小血管及出血方面比常规序列更敏感。结论SWI对于脑星形细胞瘤内静脉和出血方面显示清晰,有助于脑星形细胞瘤的分级。Objective To evaluate the value of susceptibility--weighted imaging(SWI) in grading astrocytomas of brain. Methods 22 patients with astrocytoma including grade Ⅰ - Ⅱ in 9 and grade Ⅲ~Ⅳ in 13 were examined with GE 3.0T MRI scanner. Conventional imaging techniques(T1 WI, T2 WI, FLAIR, CE- T1 WI) and SWI sequence were used. The parameters of SWI sequence were followed: TR= 35 ms,TE= 20 ms, FA= 15°, slice thickness= 2 ram. The venous structures and blood products inside the tumors on SW images were analyzed. The difference of the two groups in SW images were analyzed with statistics method. Results The findings on SW images of brain astrocytoma were correlated well with pathology. SWI was more sensitive than conventional imaging techniques in showing venous structures and blood products in brain astrocytomas,and there were significant differences. Conclusion SWI is useful in showing characteristics of brain astrocytoma,and it is valuable in grading the tumors.
