E-cadherin和LI-cadherin在早、中期胃癌中的表达及其与预后的关系

摘　　要：目的探讨胃癌组织中E-cadherin和LI-cadherin表达特点及其与胃癌临床病理特征和预后的关系。方法采用免疫组化S-P法检测215例胃癌和60例非癌组织中的E-cadherin和LI-cadherin的表达水平,并结合临床资料进行分析。结果E-cadherin和LI-cadherin在胃癌中异常表达率分别为53.0%和80.5%,且两者在胃癌前病变中也存在异常表达。E-cadherin和LI-cadherin在胃癌中的表达存在正相关(Kappa=0.21,P<0.01),与胃癌的组织分化程度(P<0.001)、Lauren分型(P<0.001)及淋巴结转移密切相关(P<0.05)。E-cadherin正常表达具有显著的生存优势(P=0.018),是预测胃癌预后的独立因素。结论E-cadherin和LI-cadherin的异常表达广泛存在于胃癌及胃癌前病变中,E-cadherin表达减少和LI-cadherin表达增加与胃癌的分化和淋巴结转移有关。E-cadherin异常表达可能是对胃癌生存期具有预测价值的重要指标。Objective To investigate the expression of E-cadherin and LI-cadherin in gastric carcinoma and to analyze the relationship of their expression with clinicopathologic factors and prognosis. Methods Two hundred and fifteen patients with early and middle stage gastric carcinoma who underwent surgical resection at the department of surgery, the First Affiliated Hospital of Anhui Medical University were followed up. Expression of E-cadherin and LI- cadherin were examined by S-P immunohistochemistry in two hundreds and seventy-five archival paraffin-embedded tissue specimens of gastric carcinoma and control groups. Results The abnormal expression rates of E-cadherin and LI-cadherin in gastric carcinoma were 53.0% and 80.5% respectively and they were also expressed abnormally in precancerous lesions. There was a positive correlation between the expression of E-cadherin and LI-cadherin （ Kappa = 0. 21 ,P 〈 0. 01 ）. The expression of E-cadherin and LI-cadherin were correlated with pathological differentiation （P 〈 0. 001 ）, Lauren grouping（ P 〈 0. 001 ） and lymph node metastasis （P 〈 0. 050）. Better survival was noted in tumors retaining normal expression of E-cadherin （P = 0. 018 ）. Conclusion Abnormal expression of E-cadherin and LI-cadherin frequently occurs in gastric carcinoma and precancerous lesions. Low expression of E-cadherin and high expression of LI-cadherin may be associated with differentiation and lymph node metastasis. In particular, ab- normal expression of E-cadherin may be a useful prognostic marker for the survival term of gastric carcinoma.
