基质金属蛋白酶-9与脑梗死相关性研究

摘　　要：目的通过观察脑梗死患者血清基质金属蛋白酶-9(MMP-9)水平的动态变化与梗死面积、神经损害的关系,从临床方面探讨两者在脑梗死发展过程中的作用。方法对70例脑梗死患者第3天和病后14 d的血清采用酶联免疫法测定基质金属蛋白酶-9,进行不同时程和同期神经功能缺损程度、脑梗死体积进行相关分析,并与30例正常人对照。结果 70例脑梗死患者第3天时血清MMP-9浓度均显著高于对照组,P<0.05。脑梗死患者血清MMP-9水平CTV≥10 cm^3,与CTV<10 cm^3比较P<0.05。不同程度神经功能受损患者血清MMP-9水平CSS>30与CSS≤30之间比较P<0.05。结论 MMP-9参与脑梗死的病理过程,脑梗死急性期血浆MMP-9水平可作为判断梗死面积大小、病情轻重及预后的客观指标。Objective Observe the dynamic changes of MMP - 9 in patients with cerebral infarction and the relation of MMP -9 leves to infarct size and intensity of neurological deficit, the MMP -9 in plasma were detected to investigation the effect of MMP - 9 to the time course of in isehemic stroke. Methods By ELISA MMP - 9 concentration in plasma were measured in 70 patients with acute cerebral infaretion（ACI）, on days 3 and 14 after onset and 30 control subjects. At the same time, Canadian Stroke Scale （CSS） score and cerebral infarction volume （CTV） were also obtained and analyzed. Results Mean plasma MMP - 9 concentrations in 70 patients significantly elevated compared with control subjects on day 3, P 〈 0.05. Moreover Mean plasmaMMP -9 concentrations in patients with CTV ≥ 10 cm3 , CSS 〉 30 were significantly higher than those with CTV 〈 10 cm3 , CSS≤30 ,P 〈0.05. Conclusion MMP -9 is involved in the pathologic process of cerebral infarction. The concentrations of MMP - 9 may be helpful as an objective guide to judging the degree, size of infarction area and the recerlt prognosis in acute cerebral infarction.
