摘　　要：目的应用螺旋CT双期增强扫描,探讨自身免疫性胰腺炎CT征象。方法 CT增强扫描25例自身免疫性胰腺炎患者,评价自身免疫性胰腺炎胶囊样环,胰周线征,胰腺钙化,胰腺假囊肿,胆管壁增强,肾脏损害,胰管扩张与突然截断出现频率,确定在自身免疫性胰腺炎中诊断价值。结果自身免疫性胰腺炎CT增强后胰腺呈弥漫性低密度17例(68%),胰腺局限性肿块且增强后呈低密度10例(40%),胶囊样环16例(64%),胰周线征15例(60%),胰周血管受累11例(44%),胰腺病灶钙化8例(32%),胰腺假囊肿3例(12%),胰管扩张5例(20%),胰管截断4例(16%),胆管扩张14例(56%),胆管壁增强19例(76%),腹膜后纤维化4例(16%),肾脏损害12例(48%)。结论自身免疫性胰腺炎CT增强呈现弥漫性肿大且呈低密度,胶囊样环,胆管壁增强,肾脏损害是诊断自身免疫性胰腺炎重要CT征象。Objective Application of spiral dual - phase CT enhanced scan to explore the CT signs of autoimmune pancreatitis. Methods 25 cases of enhanced CT scanning in patients with autoimmune pancreatitis, Evaluation of autoimmune pancreatitis capsule - like rim, peripancreatic strands, and pancreatic calcifications ; pancreatic duct or bile duct changes ; and renal involvement. Determine the diagnosis of autoimmune pancreatitis in value. Results Autoimmune pancreatitis enhanced CT showed diffuse low density in 17 cases （68%） , Limitation of pancreatic mass and enhanced low -density was 10 cases （40%） , the presence of a capsule - like rim16cases（64% ）, peripancreatic strandslScases（ 68% ） , Peripancreatic vascular involvement in 11 cases （44%）, Calcification in 8 cases of pancreatic lesions （32%） , Three cases of pseudocyst （ 12% ） , 5 cases of pancreatic duct expansion （20%） , Truncated four cases （ 16% ） , 14 cases of biliary dilatation （56%） , 19 cases of bile duct wall enhancement （76%） , 4 cases of retroperitoneal fibrosis （ 16% ） , 12 cases of renal injury （48%）. Conclusion The Autoimmune pancreatitis enhanced CT showed diffusely decreased pancreatic enhancement, a capsule - like rim,a bile duct wall enhancement and a renal injury is an important diagnosis of autoimmune pancreatitis CT signs.
