摘  要:目的探讨低氧诱导因子-1α(HIF-1α)、血管内皮生长因子-C(VEGF-C)在非小细胞肺癌(NSCLC)中的表达及其与淋巴转移的关系。方法采用PV-9000免疫组织化学法检测58例NSCLC和20例癌旁对照肺组织中HIF-1α、VEGF-C的表达情况,并记数VEGFR-3标记淋巴管内皮细胞的微淋巴管密度(LMVD)。结果HIF-1α和VEGF-C在NSCLC组的阳性表达率分别为65.5%和60.3%,明显高于癌旁对照肺组织(P<0.05);NSCLC组织中HIF-1α和VEGF-C的表达的高低与有无淋巴结转移、TNM临床分期密切相关(P<0.05),而与性别、年龄、病理组织类型以及肿瘤的分化程度无显著相关(P>0.05);LMVD在NSCLC组和癌旁对照组计数分别为6.6±2.4和2.9±1.7,差异有统计学意义(P<0.05);HIF-1α与VEGF-C的表达呈明显相关(rpearson=0.464,P<0.05),HIF-1α与LMVD呈正相关(rs=0.506,P<0.05),VEGF-C与LMVD也呈正相关(rs=0.608,P<0.05)。结论HIF-1α及其靶基因VEGF-C在NSCLC中的表达明显上调,HIF-1α可能通过诱导VEGF-C的过表达参与NSCLC的淋巴管生成,在NSCLC的发生、发展以及转移过程中发挥着重要作用。Objective To investigate the expression of HIF - 1α,VEGF - C and their relation to lymphangiogenesis and lymph node metastasis in non small cell lung cancer(NSCLC). Methods The expressions of HIF - 1α and VEGF - C in 58 cases of NSCLC tissues and 20 cases of the adjacent tissues were detected by PV - 9000 immunohistochemical technique. LMVD was determined by VEGFR -3 immunostaining. Results The positive rate of HIF- 1α and VEGF- C expression in NSCLC was 65.5% and 60.3% ,which is significantly higher than that in the adjacent tissues ( P 〈 0.05 ). There was a significant difference of HIF - 1α and VEGF - C protein expression between NSCLC tissues and the adjacent tissues ( P 〈 0.01 ). The expression of HIF - 1α and VEGF - C were both correlated with lymph node involvement ( P 〈 0. 05 ) and TNM clinical stage ( P 〈 0. 05 ), but no significant correlation between sex, age, pathological type and differentiation ( P 〉 0. 05). The expression of HIF - 1α was correlated with VEGF - C expression and LMVD ( P 〈 0.05). Conclusion Over expression of HIF - 1α induces the tumor lymph angiogenesis in NSCLC by activating the transcription of VEGF - C gene . It may play an important role in the generation, development and lymphatic metastasis in NSCLC.
