胃溃疡患者血清胃动素、肽YY、IgG和铁蛋白的研究

摘　　要：为探讨胃溃疡患者血清胃动素(MTL)、肽YY(PYY)、免疫球蛋白G(IgG)及铁蛋白(FRT)含量及变化的临床意义,选胃溃疡患者18例,男11例,女7例,平均46.6岁。均经纤维胃镜检查和X线钡餐检查确诊,并排除胃肠道肿瘤和内分泌疾病。病变部位:胃窦部12例、胃底部3例、胃体部3例。按胃壁分布:胃小弯14例、胃大弯2例、前壁1例和后壁1例。按胃溃疡的病期分为活动期(A期)13例和愈合期(H期)5例。另选健康对照者26例,男17例,女9例,平均44.4岁。均于确诊后3日内的上午07∶00时空腹采血,用放射免疫法测定血清MTL、PYY、IgG和FRT。与对照者比较,胃溃疡患者血清MTL(207.32±98.45对316.29±98.36ng/L,P<0.01),PYY(1.09±0.64对1.71±0.84pmol/L,P<0.01)和IgG(13.06±6.97对25.31±10.08g/L,P<0.01)明显升高,而FRT(98.37±41.52对127.12±64.06μg/L,P>0.05)升高不明显。MTL与PYY升高呈正相关(r=0.4436,P<0.05)。胃溃疡发生部位之间上述血清各值无明显差异(P>0.05),A期血清MTL和PYY较H期高,但无统计学差异(P>0.05)。10例胃溃疡患者临床治愈后复查,仅IgG下降(P<0.05),而FRT、MTL和PYY无明显变化。结果提示胃溃疡时患者血清MTL、PYY、IgG和FRT可以发生改变,测定这些物质对研究胃溃疡时的内分泌变化有一定参考价值。To determine the clinical significance of the changes of serum levels motilin(MTL), peptide YY (PYY),immunoglobulin G(IgG)and ferritin(FRT)in patients with gastric ulceration (GU), a clinical study was performed. 18 patients with GU,consisting of 11 men and 7 women,and including ulceration in the antrum( n = 12),in the body(n. = 3),and in the fundus( n = 3)of the stomach,have been enrolled in the study. 26 healthy subjects were investigated as controls. Serum levels of MTL, PYY, IgG and FRT were measured with radioimmunoassay kit. For patients with GU,serum MTL(207.32 ± 98.45 vs 316.29 ±98.36ng/L,P<0.01),PYY(1.09±0.64 vs 1.71±0.84pmol/L,P<0.01)and IgG( 13.06 ± 6.97 vs 25.31 ± 10.08g/L,P<0.6l)were much higher,and FRT(98.37 ± 41.52 vs 127.12 ± 64.06μg/L,P >0.05)was unchanged,than those in normal subjects.The serum MTL level related with the PYY(r = 0.443 6,P<0.05). A follow-up in the 10 cases cured,serum IgG( 16.36 ± 7.28 vs 26.58 ± 10.14g/L,P <0.05)was significantly lowered,and MTL,PYY and FRT had no change, than those of the pre-treat-ment. These data showed that the serum levels of MTL, PYY, IgG,FRT were increased in patients with GU,and they were of clinical significance for study gastric ulceration.
