摘  要:目的探讨原发性癫痫患者治疗前后血清细胞因子白细胞介素-2(IL-2)、白细胞介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)水平变化及其与局部脑血流量(rCBF)变化的相关性。方法采用酶联免疫吸附法(ELISA)检测22例原发性癫痫患者治疗前、抗癫痫治疗3个月后、对照组血清IL-2、IL-6、TNF-α、rCBF。结果癫痫患者治疗前血清IL-2、IL-6及TNF-α水平明显高于对照组(P<0.01),而rCBF明显低于对照组(对照组未发现rCBF下降);治疗后血清IL-2、IL-6及TNF-α水平明显低于治疗前(P<0.01),而rCBF明显高于治疗前。结论癫痫患者存在细胞免疫功能紊乱,临床药物治疗对细胞因子水平有影响。rCBF检查是一种检测癫痫的敏感技术,发作间期rCBF检查可有效预测疗效。Objective To observe the change of varied cytokines interleukin(IL)-2,interleukin(IL)-6 and tumor necrosis factor alpha(TNF-α) and their correlation with regional cerebral blood flow(rCBF) before and after treatment. Methods ELISA was used to measure serum IL-2,IL-6 and TNF-α before treatment and after 3 month treatment in 22 cases of primary epilepsy and 18 cases in control group. The data of detection results were compared in two groups. Results The levels of IL-2 ,IL-6 and TNF-α in the epilepsy group before treatment were significantly higher than those in the control group(P(0.01), but rCBF was obviously lower. The levels of IL-2,IL-6 and TNF-α in the epilepsy group after treatment were lower than those before treatment, while rCBF signif icantly higher than before treatment. There was a positive correlation between cytolines (IL-2, IL-6 and TNF-α) and rCBF(P( 0.01). Conclusion The patient with epilepsy has cytommunity disorder. Drug treatment affects the cytokines levels, rCBR determination is a sensitive technique to detect epilepsy and rCBF performed in epileptic seizure can predict the treatment curative effective- ly.
