进展期胃癌CT影像学特征与组织学分化的相关性初探

摘　　要：目的研究多层螺旋CT(multicle spiral CT,MSCT)增强扫描表现与胃癌组织学分化的相关性。方法采用MSCT对47例经手术病理证实为胃癌患者术前行低张力水充盈螺旋CT三期增强扫描,并将肿瘤之CT征象(CT三维图像上肿瘤最大径及厚度、强化方式及程度、瘤周低密度带厚度)与术后病理进行对照,对所得数据行统计学处理。结果(1)MSCT三维成像上病灶厚度、强化方式、强化程度及瘤周低密度带厚度与胃癌分化程度密切相关(P<0.05);(2)MSCT三维成像上肿瘤最大径与分化程度相关性不显著(P>0.05)。结论MSCT增强扫描部分表现与胃癌分化程度密切相关,对于术前诊断胃癌的分化程度、分析术式及评价预后具良好价值。Objective To investigate the correlation of spiral CT findings with histological differentiation in advanced gastric carcinoma. Methods Triphasic spiral CT scans with water filling method and administration of hypotonic agent were performed in 47 patients with advanced gastric carcinoma. The CT findings （including maximal diameter and thickness of tumor in three dimensional picture of CT image,enhanced features and degree of enhancement,thickness of peritumor low- density band） were compared with histological differentiation of cancer confirmed by surgical pathology. Results The thickness of tumor in three dimensional picture of CT image, enhanced features and degree of enhancement, thickness of peritumor low-density band were significantly correlated with tumor histological differentiation （P〈0.05） ; Maximal diameter of tumor in three dimensional picture of CT image showed no correlation with tumor histological differentiation. Conclusion The preoperational spiral CT findings are correlated with the histological differentiation in postoperative pathology of advanced gastric carcinoma.
