肝静脉频谱波形分析对脂肪肝诊断的临床价值研究

摘　　要：目的利用脉冲多普勒评估肝静脉(HV)频谱波形对脂肪肝临床应用中的价值。方法利用脉冲多普勒超声评估50例正常人和93例不同程度脂肪肝患者HV频谱波形(HV0型、HVⅠ型、HVⅡ型)。结果随着脂肪肝程度的加重,HVⅠ型和HVⅡ型明显增多。脂肪肝各组的HVⅠ+HVⅡ比例均高于对照组,差异均有统计学意义(χ2分别=10.59、30.80、24.91,P均<0.05),且中度、重度脂肪肝组的HVⅠ+HVⅡ比例高于轻度脂肪肝组,差异也均有统计学意义(χ2分别=7.39、5.23,P均<0.05),但重度和中度脂肪肝组之间比较,差异无统计学意义(χ2=0.27,P>0.05)。结论脉冲多普勒超声HV频谱波形分析有助于脂肪肝的诊断和分级。Objective To evaluate the clinical diagnostic value of the Doppler waveform spectrum of hepatic vein （HV） in patients with fatty liver. Method Fifty healthy controls and 93 patients with fatty liver underwent spectral Doppler sonography . The flow pattern of the HV was subsequently classified as type HV0, HV Ⅰ ,and HVⅡ. Results With the aggravation of the disease, type HV Ⅰ and HV Ⅱ appeared more frequently. There were differences in Doppler waveforms of hepatic vein between the controls and the patients classified by the severity of fatty liver respectively （X^2=10.59, 30.80, 24.91 ,P〈0.05）;there were differences in Doppler waveforms of hepatic vein between the patients with mild, moderate and severe fatty liver （X^2=7.39, 5.23, P〈0.05）; there was no difference between the moderate and severe group （X^2=0.27 ,P〉0.05）. Conclusions The HV Doppler waveform spectral sonography may be useful for the diagnostic evaluation of the fatty liver.
