芬太尼透皮贴剂治疗高龄癌痛患者的临床疗效观察

摘　　要：目的:探讨高龄(≥75岁)中重度癌痛患者接受芬太尼透皮贴剂治疗的止痛疗效及安全性。方法:449例中重度癌痛患者按年龄随机分为三组:A组≥75岁,B组60～74岁,C组<60岁。均使用芬太尼透皮贴剂治疗,初始剂量从25ug/h开始,连续使用15d后评价疗效、生活质量变化及不良反应。结果:所有病人用药后止痛效果明显,总有效率为87.3%;其中A组为87.5%;B组为86.4%;C组为88.3%。三组之间总有效率差异无显著性(P>0.05)。生活质量均明显提高。不良反应轻,主要为:头晕,嗜睡,便秘,恶心,呕吐等。三组中的不良反应差异无显著性(P>0.05)。结论:芬太尼透皮贴剂治疗高龄(≥75岁)中重度癌痛患者方便,安全,有效,能提高生活质量,经济实用,值得临床进一步推广应用。Objective： To evaluate the effects and side effects of transdermal fentanyl for the elderly patients （ age ≥ 75 ） with moderate to severe cancer pain. Method： 449 patients with moderate to severe cancer pain were randomly divided into three groups . Group A was age over 75 years, group B was age in 60 - 74 years, and group C was age less 60 years. All patients received transdermal fentanyl to treat cancer pain with the initial dose began at 25ug/h. Each patient used transdermal fentanyl for 15 days at least. They were asked to record the attacks of pain, quality of life, and any side effects of the treatment. Result： After treatment, the over all analgesic response rates were 87.3% ,in group A were 87.5% ,group B were 86. 4% ,and group C were 88.3%. There was no s;oo-nificant difference between the three groups （ P 〉 0.05 ）. the quality of life of all patients was improved obviously. The side effects of both group were mild, the side effects included dizziness, sleepiness, constipation, nausea, vomiting and or so. The side effects incidence rate in each group was equal nearly. There was no significant difference among the three groups（ P 〉 0.05）. Conclusion：It＇s safe, convenient and capable to improve the quality of life,transdermal fentanyl is effective and worth recommending as a first - line drug for the treatment of elderly patients （ age ≥ 75 ） with moderate to severe cancer pain. It can be recommended widely to clinical use.
