摘 要:目的:观察三维适形放射治疗(3DCRT)、热疗联合中药狼毒灌注综合治疗对中晚期肺癌的临床疗效,放疗量效关系以及热疗次数与疗效关系。方法:185例患者中,3DCRT放射治疗54例,3DCRT联合热疗63例,3DCRT、热疗联合中药狼毒灌注综合治疗68例。治疗结束后,统计分析各组临床疗效的差异,并分析放疗和热疗量效之间的关系。结果:3DCRT、热疗联合中药狼毒灌注组有效率80.88%,单纯3DCRT治疗有效率42.59%,3DCRT联合热疗有效率57.14%;放射治疗量DT<40Gy有效率40.00%,DT40~60Gy有效率60.00%,DT≥60Gy有效率为76.67%;内生场治疗<4次有效率为42.86%,≥4次有效率70.67%。经统计学处理,各组疗效差异有显著性(P<0.05)。结论:3DCRT、热疗联合中药狼毒灌注综合治疗组能更有效的控制肿瘤发展;在病人身体能耐受的前提条件下,放射剂量与疗效呈正相关;内生场治疗次数与疗效呈正相关。Objective: To observe the three-dimensional conformal radiation therapy (3DCRT) ,thermotherapy combined with traditional Chinese medicine integrated chamaejasme reperfusion therapy in advanced lung cancer, radiation therapy dose-effect relationship and hyperthermia dose-effect relationship. Methods: 185 patients, 3DCRT treatment of 54 cases of radiation, 3DCRT joint 63 cases of hyperthermia, 3DCRT, chamaejasme hyperthermia combined with Chinese medicine treatment of 68 cases of the integrated perfusion. After treatment, statistical analysis of group differences in clinical efficacy and analysis of radiation and hyperthermia dose-effect relationship between. The results: 3DCRT, hyperthermia combined with tradi- tional Chinese medicine reperfusion group chamaejasme 80.88% efficient, simple and efficient 3DCRT treatment 42.59%, 3DCRT combined hyper-thermia 57.14% efficient; radiation therapy volume DT 〈40Gy efficiency 40.00%, DT40 - 60Gy efficient 60.00%, DT ≥ 60Gy to 76.67% efficient; the treatment of endogenous field〈4times the effective rate was 42.86%, ≥ 4 times 70.67percent efficient. After statistical processing, effect of each group there were significant differences (P 〈0.05) . Conclusions: 3DCRT, thermotherapy, chamaejasme reperfusion therapy combined group to more effectively control tumor development; in the patient's body can be a prerequisite for tolerance, the radiation dose was positively correlated with the effect; endogenous field treatment are positively correlated with efficacy.
